摘要
目的探讨经尿道2μm激光前列腺剜除术治疗BPH的疗效和安全性。方法比较分析2009年4月至2011年3月接受经尿道2μm激光前列腺剜除术治疗的190例BPH患者术前、术中及术后相关临床指标,随访时间长达3年。结果手术平均切除前列腺组织(23.96±15.8)g,平均手术时间(70.5±11.6)min,术中平均出血量(20.2±16.9)ml。所有患者手术前后血钠浓度及血红蛋白水平均无明显变化,国际前列腺症状评分、生活质量评分及最大尿流率均较术前有明显改善。3例患者术后出现继发性尿失禁,经功能锻炼3个月后康复。结论经尿道2μm激光前列腺剜除术具有良好的止血效果,切除增生前列腺组织更彻底,手术风险显著降低,是治疗BPH安全、有效的微创手术方式,疗效持久、稳定。
Objective To investigate the safety and efficiency of transurethral 2μm laser enu-cleation in treating the benign prostatic hyperplasia（BPH）. Methods 190 cases of BPH were di-vided into two groups randomly.They were underwent transurethral prostate enucleation with 2μm laser in our hospital from April 2009 to March 2011.The pre,mid,and postoperative clinical inde-xes were collected and analyzed. Results The amount of removed prostatic tissue was （23.96± 15.8）g.The average operational duration was （70.5±11.6）min.The mean blood loss during opera-tion was （20.2±16.9）ml.There were no significant change in serum sodium concentration and he-moglobin levels before comand after surgery.The international prostate symptoms score （IPSS）, quality of life （QOL）and maximal urinary flow rate （Qmax）was improved significantly after opera-tion.3 patients developed into the secondary urinary incontinence and then recovered in 3 months lat-er through functional exercises. Conclusions The transurethral prostate enucleation with 2μm la-ser in treating the BPH has more radical resection of hyperplastic prostate tissue,less blood loss. The transurethral prostate enucleation with 2 μm laser is a safe and effective minimally invasive treatment for BPH,and showing the stable and sustained curative effects.
