摘要
目的探讨经皮冠状动脉介入治疗对冠心病合并左心功能不全患者心功能的影响。方法选取2012年4月—2014年1月益阳市中心医院收治的冠心病合并左心功能不全患者88例,按治疗方法不同分为试验组45例和对照组43例。试验组采用经皮冠状动脉介入治疗,对照组采用常规药物治疗。比较两组患者治疗前和治疗后6个月左心功能指标〔左室收缩末内径(LVDs)、左室短轴缩短率(FS)、左室舒张末内径(LVDd)及左室射血分数(LVEF)〕、纽约心脏病协会(NYHA)分级及治疗期间并发症发生情况。结果治疗前两组患者LVDs、FS、LVDd、LVEF及NYHA分级比较,差异无统计学意义(P>0.05);治疗后6个月试验组患者LVDs和LVDd低于对照组,FS和LVEF高于对照组,NYHA分级优于对照组(P<0.05)。试验组患者并发症发生率与4.4%,对照组为0,差异无统计学意义(P>0.05)。结论经皮冠状动脉介入治疗能有效改善冠心病合并左心功能不全患者的心功能,提高患者生命质量。
Objective To investigate the impact of percutaneous coronary intervention （PCI） on heart function of coronary heart disease patients complicated with left heart dysfunction. Methods A total of 88 coronary heart disease patients complicated with left heart dysfunction were selected in the Central Hospital of Yiyang from April 2012 to January 2014, and they were divided into experiment group （ n = 45 ） and control group （ n = 43 ） according to therapeutic methods. Control group given conventional medical treatment, while experiment group given PCI. Heart function index including left ventficular end - systolic dimension （LVDs） , fraction shortening （ FS）, left ventricular end - diastolic diameter （LVDd） , left ventricular ejection frac- tion （LVEF）, NYHA grading before and ＇after 6 months of treatment, and incidence of complications during treatment were com- pared between the two groups. Results There was no significant differences of EVDs, FS, LVDd, LVEF and NYHA grading between the two groups before treatment, but LVDs and LVDd were lower, FS and LVEF higher, NYHA grading of experiment group better than those of control group after 6 months of treatment. The incidence of complications of experiment group was 4.4%, that of control group was 0, the difference was not significantly different （ P 〉 0.05 ）. Conclusion PCI can effectively improve the heart function and quality of life of coronary heart disease patients complicated with left heart dysfunction.
