利用微种植支抗钉改善露龈微笑的效果评价 被引量:4

Effect of one microscrew anchorage on improving gummy smile

摘要 目的观察仅在上颌两颗中切牙之间植入微种植钉即刻负载矫治露龈微笑的临床效果。方法选择露龈微笑拔牙正畸患者10例,在其上颌唇侧两颗中切牙之间的根尖牙槽骨处植入微种植钉,即刻用链状皮圈将微种植钉和主弓丝连在一起,施力80g^100g,同时利用后牙区微种植支抗钉内收前牙,每侧施力200g,每4周复诊1次,通过X线头影测量及临床观察,比较露龈微笑矫治前后有关参数变化情况。结果露龈微笑矫治(5.0±3.5)个月,正畸矫治疗程(19.5±4.0)个月,露龈微笑得到改善,面型与矫治前比较得到很大改善,压入前后有较大变化的指标有SNA,ANB,MP-FH,U1-SN,U1-L1,U6/PP,U1/PP,L1/MP,Overjet,Overbite,U1-E1,L1-E1。而压入后期治疗仍有显著变化的有MP-FH,U1-L1,U6/PP,L1/MP,Overjet,前牙覆牙合在停止压入时测量值接近正常,但在后续矫治中有少量复发。结论微种植体支抗即刻负载,效果可靠,操作简单,创伤小,易接受,是矫治露龈微笑的理想支抗选择。 Objective To research the improving efficacy of one minroscrew anchorage for gummy smile.Methods 10 patients with gummy smile were choosen to implant microscrews into the alveolar bone between the roots of 11 and 21,connected the microscrew and the archwire with plastic chain using 80 g -100g force,200 g force each side was applied immediately for anterior teeth retraction by implant microscrews in the posterior area of arch. Results After an average period of(5.0 ±3.5) months' treatment,the gummy smile was improved. The total period of treatment is(19.5±4.0)months there were significantly changes in SNA,ANB,MP-FH,U1-SN,U1-L1,U6/PP,U1/PP,L1/MP,Overjet,Overbite,U1-E1,L1-E1 compared with those pretreatment. There were significant changes in MP-FH,U1-L1,U6/PP,L1/MP,Overjet in the later treatment. But overbite was relapes in later treatment. Conclusion Microscrew could be an effective method for improving gummy smile.
