双源CT低剂量腹部扫描在肝癌诊断中的应用 被引量:7

摘要 目的探讨肝癌患者腹部扫描诊断中应用低剂量CT诊查的临床价值,为临床诊断提供参考。方法选择2011年12月-2013年12月浙江省立同德医院住院部CT室收治的肝癌患者48例,初次CT诊查应用常规剂量进行,复查分别使用70%、50%低剂量CT诊查。对比不同剂量CT诊查图像结果及射线剂量。结果常规剂量诊断图像治疗优良率为95.83%,70.0%剂量诊断图像治疗优良率为93.75%,50.0%剂量诊断图像治疗优良率为89.58%。图像优良率大致呈降低趋势,但三种剂量扫描图像质量相近,差异无统计学意义(P〉0.05);常规剂量噪声为(11.76±0.35),70%剂量噪声为(12.03±0.44),50%剂量噪声为(12.42±0.52),三组扫描噪声值相近,差异无统计学意义(P〉0.05)。常规剂量辐射剂量水平为(96.59±0.82)m Gy,70%剂量辐射剂量水平为(67.74±0.93)m Gy,50%剂量辐射剂量水平为(48.17±0.91)m Gy,随着扫描剂量降低,辐射水平明显降低,低剂量扫描辐射水平与常规剂量对比差异均有统计学意义(P〈0.05)。结论肝癌患者腹部扫描应用CT诊查时,低剂量CT诊查结果可信,辐射剂量小,可优先考虑使用。 Objective To investigate the clinical value of low dose CT inspecting in diagnosis of abdominal scan of patients with liver cancer, and to provide reference for clinical diagnosis. Methods Forty eight cases of patients with liver cancer admitted to CT Room of Inpatient Department in Zhejiang Provincial Tongde Hospital were chosen. The patients examined first by CT used routine dose, and used 70%, 50% low dose to check at reexamination. The image result and roentgendosis of different dosages of CT inspecting were compared. Results The good rate of diagnostic image with conventional dose was 95.83%, which of diagnostic image with 70.0% dose was 93.75%, which of diagnostic image with50.0% dose was 89.58%, the good rate of diagnostic image tended to decrease substantially, but the image quality of three-dose scan was similar, the difference was not statistically significant(P〉0.05); the noise of conventional dose was(11.76±0.35), which of 70% dose was(12.03±0.44), which of 50% dose was(12.42±0.52), the noise level of threedose scan was similar, the difference was not statistically significant(P〉0.05). The radiation dose level of conventional doses was(96.59±0.82) m Gy, which of 70% dose was(67.74±0.93) m Gy, which of 50% dose was(48.17±0.91) m Gy.With reduction of scan doses, radiation levels were significantly decreased, there were statistically significant differences between low-dose radiation level and conventional-dose radiation level(P〈0.05). Conclusion When CT inspecting is used for abdominal scan of patients with liver cancer, results of low dose CT inspecting are reliable, the radiation dose is small, which can be given priority to use.
