摘要
目的 探讨冠状动脉慢血流(CSF)患者血小板形态及功能的改变.方法 经冠状动脉造影(CAG)诊断为CSF者38例,CAG显示无管腔狭窄且无慢血流者25例为对照组,使用校正的TIMI血流分级(CTFC)方法评价冠状动脉的血流速度.两组均采用ELISA法测定血小板α颗粒膜蛋白-140 (GMP-140)的浓度;进行血常规检验,测定血小板计数(PLT)、血小板平均体积(MPV)、血小板体积分布密度(PDW).比较两组GMP-140、PLT、MPV的水平,分析冠状动脉血流速度与GMP-140及MPV的相关性.结果 ①CSF组平均血流速度明显慢于对照组.②CSF组GMP-140及MPV水平明显高于对照组.③冠状动脉平均血流速度与GMP-140及MPV均呈正相关.结论 血小板体积增大、聚集功能增强,提示冠状动脉慢血流患者处于较高的血小板活化状态,可以加速冠状动脉粥样硬化病变的发生发展.
Objective To compare the changes of the morphology and function of platelets in patients with coronary slow flow phenomenon(CSF).Methods Thirty-eight patients with CSF and 25 control cases with normal coronary flow (NCF) were enrolled in the study.Coronary flow speed of the cases were determined by thrombolysis in myocardial infarction (TIMI) frame count method.The platelet count (PLT),mean platelet volnme (MPV),and the platelet volume distribution width(PDW) were evaluated by blood routine examination.The CSF,NCF and GMP-140 were compared to with evaluate the relationship between coronary flow speed and MPV (mean platelet volume),coronary flow speed and GMP-140.Results Coronary flow speed of the patients with CSF was much slower than that of control group.The level of GMP-140 and MPV were much higher than that of control group.Coronary flow speed was positively associated with the GMP-140 and MPV.Conclusions The MPV increasing and the aggregation reinforcing indicate that the platelets of CSF patients in the state of activation,it may accelerate the progress of atherosclerosis.
