摘要
目的探讨负压封闭引流技术(VSD)联合抗生素干预在跟腱术后感染患者中的临床效果。方法选取2009年3月—2013年2月我院骨科收治的跟腱术后感染伴皮肤缺损患者100例。对2009年3月-2011年2月收治的50例患者(对照组)采取传统治疗,对201 1年3月-2013年2月收治的50例患者(观察组)采取VSD技术联合抗生素干预治疗。观察并比较两组患者治疗后的疗效,记录两组治疗后的感染控制、肉芽组织生长情况及首次植皮成功率。结果观察组患者治疗后优良率为98%,明显高于对照组(76%);观察组患者治疗后创面愈合时间及住院时间分别为(17.6±4.4)d、(24.5±6.1)d,均明显少于对照组的(23.3±5.9)d、(36.8±7.6)d;观察组患者感染控制率、创面肉芽组织生长良好率及首次植皮成功率分别为100%、100%、98%,明显高于对照组(72%、68%、60%),差异具有统计学意义(P均<0.05)。结论对跟腱术后感染伴皮肤损伤的患者行负压封闭引流术联合抗生素治疗的疗效显著,感染得到有效控制,VSD技术应用后可改善局部微循环,促进肉芽组织的增生,局部增殖的细菌得以控制,是一种操作简单、疗效好的治疗手段,且利于二期植皮。
Objective To explore the clinical effect of vacuum sealing drainage （VSD） combined with antibiotic intervention on the postoperative infection of Achilles＇ tendon. Methods A total of 100 patients with postoperative infection of Achilles ＇ tendon plus skin defects treated during March 2009 and February 2013 were enrolled and divided into two groups. Control group （n = 50） was treated with conventional therapy during March 2009 and February 2011, and trial group （ n = 50） with VSD technology plus antibi- otic intervention during March 2011 and February 2013. Results The excellent rate, healing time and hospitalization time of the trial group and the control group were 98% vs 76% , 17.6±4.4 vs 23.3 ±5.9 d and 24.5 ±6.1 vs 36.8 ±7.6 d, and the infection control rate, good granulation tissue growth rate and initial skin graft success rate of the two groups were 100% vs 72% , 100% vs 68% and 98% vs 60% , respectively. There were significant differences in all the above parameters （ P 〈 0.05 ）. Conclusion VSD in combination with antibiotic intervention is clinically effective on the postoperative infection of Achilles＇ tendon with local skin defects. It may improve mierocireulation, promote granulation tissue proliferation and control infection.
