摘要
目的 探讨眼底血管样条纹（angioid streaks,AS）患者在在频域光相干断层扫描（fourier-domain optical coherence tomography,FD-OCT）中的图像特征.方法 对2010年1月至2011年12月在河北省眼科医院就诊的13例26只眼经临床确诊的眼底血管样条纹患者,采用日本Topcon公司的3D OCT-1000仪器进行检查,详细观察患者眼底后极部在FD-OCT中的各种细微改变.结果 13例26只眼患者后极部的Bruch膜（bruch membrane,BM）反光带在OCT图像中均出现不同程度的增厚、反射增强,并呈波浪状改变.并且其上下方邻近组织结构紊乱,突出表现为13例26只眼的IS/OS层萎缩变薄或缺失;5只眼伴有脉络膜、视网膜萎缩.所有眼底条纹均表现为对应部位的BM反光带断裂.13只眼黄斑区合并有活动性CNV者,表现为神经上皮下有团块状强反光物质分布,其周围视网膜神经上皮增厚,其内或下方可见液性腔隙分布.5只眼黄斑区视网膜下有机化膜者,表现为神经上皮组织结构紊乱,被呈强反射的机化膜组织代替.还有2只眼黄斑区缺乏CNV者,OCT显示黄斑区神经上皮囊样改变.结论 AS病变在OCT检查中有特征性改变,临床医生根据这些特征能及时做出正确地诊断和治疗.
Objective To investigate the image features of angioid streaks patients on the Fourier-domain optical coherence tomography.Methods Thirteen patients（26 eyes）who diagnosed as angioid streaks in clinical were accepted optical coherence tomography （OCT） by 3D OCT-1000 of Topcon company produced in Japan,and the subtle changes of the FD-OCT images were observed detailed.Results The Bruch＆#39;s membrane（BM）of the posterior pole area were thickening,appeared as increased reflectivity and undulation of 13 patients（26 eyes）on the OCT images.The structure inward and outward of BM were disruption,predominant showed the IS/OS layer thinning or atrophy of 13 patients（26 eyes）and the chorioretinal atrophy of 5 eyes.All streaks had consistently been found that the BM broken.Thirteen eyes combined with the active CNV showed the subneuroepithelium existing clump materials of highly reflectivity,with the surrounding retina thickening and liquid acumulating within or below the neuroepithelium.Five eyes with organized membrane in macula were displayed that the tissue structural disordered and replaced by the membrane with strong reflection and some areas neuroepithelial significant atrophy thinning.Two eyes without CNV showed the cystoid macular edema on OCT images.Conclusions Angioid streaks have the characteristic changes in the OCT,the clinic doctor can make the correct diagnosis and treatment in time according to these characteristics.
