老年性黄斑变性不同分期的光相干断层扫描图像特征 被引量:6

摘要 老年性黄斑变性(AMD)以玻璃膜疣形成、视网膜色素上皮(RPE)层异常、地图样萎缩(GA)、新生血管性黄斑病变为主要特征。可分为无AMD、早期、中期和晚期AMD。后者通常又分为GA(萎缩型)和脉络膜新生血管(CNV)(渗m型)。光相干断层扫描(OCT)图像中,早期表现为玻璃膜疣小丘状中强反射隆起的病灶,伴或不伴RPE反射带和视网膜内外节反射带异常;中期表现为融合的玻璃膜疣为多个连续或不连续的丘陵状RPE反射带隆起,大小不一,内部反射从中弱到中强不等;晚期患眼GA表现为RPE层光反射减少,下方组织的反射增强,或者是黄斑区RPE反射带消失和视网膜各层反射带紊乱甚至消失,视网膜厚度变薄;CNV表现为RPE层中强反射隆起,多伴有视网膜内水肿和视网膜下积液存在。正确认识这些OCT特征,有助于AMD的临床诊断和随访观察。
