股骨转子间骨折一些临床争议与热点问题 被引量：2

摘要 股骨转子间骨折是老年人常见疾病。对股骨转子间骨折,一般推荐尽早手术治疗,以减少卧床风险,但若患者有多种内科伴发疾病,则要在术前进行适当优化调整治疗,方可实施手术。术前一般无须牵引,手术时可选择全身麻醉或神经阻滞麻醉。既往常采用Evans-Jensen分型,目前AO分型更常用。缺乏股骨距内侧柱支撑、外侧壁骨折和大转子骨折为不稳定骨折的标志。通常认为对稳定骨折采用髓外固定,对不稳定骨折采用髓内钉,慎用锁定钢板和人工关节置换。手术者应充分了解内固定物优缺点,避免出现复位不良、内固定物断裂或切出、髋内翻和内固定物周围骨折等并发症。骨折内固定术后,应尽早下地活动,减少卧床并发症。该文就股骨转子间骨折治疗上的一些争议和热点问题作一综述。
