摘要
目的：探讨肝炎及肝硬化患者凝血功能及血小板相关参数指标与疾病进展的关系.方法：收集227例肝病患者及96例健康对照者,检测样本的凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）、凝血酶时间（TT）、纤维蛋白原（FIB）水平及血小板相关参数血小板计数（PLT）、平均血小板体积（MPV）、血小板压积（PCT）、血小板分布宽度（PDW）.对肝硬化患者的凝血功能与肝功能Child-Push分级进行相关性分析.采用t检验分析各组指标的差异.结果：肝硬化组凝血功能与正常对照组之间有显著性差异（P＜0.01）,慢性肝炎组的凝血功能与正常对照组有显著性差异（PpT＜0.05、PAPTT＜0.01、PTT＜0.05、PFIB＜0.05）.肝硬化Child-Push C级的凝血功能与健康对照组有显著性差异（P均＜0.01）且与Child-Push A级有显著性差异（PpT＜0.01、PAPTT＜0.05、PTT＜0.05、PFm＜0.01）.肝硬化Child-Push B级的凝血功能与健康对照组亦有显著性差异（PPT＜0.05、PAPTT＜0.01、PTT＜0.05、PFm＜0.05）.肝硬化组的血小板总数及相关指标与健康组有显著性差异（PPLT＜0.01、PMPV＜0.05、PPCT＜0.01、PPDw＜0.01）,慢性肝炎组血小板总数及相关指标与对照组有显著性差异（PPLT＜0.058、PpCT＜0.05）.结论：凝血功能指标及血小板相关指标的检测对评价肝脏受损程度、评估肝病患者不同病程时期的凝血功能状况及患者预后评估等均具有重要意义.
Objective ： To assess the correlationship of coagulation function and parameters of platelet and the progress of liver disease. Methods ： 96 health subjects and 227 liver patients were enrolled to detect PT, APTT, TT, FIB, and parameters of platelet（PLT, MPV, PCT, PDW）. In addition, the correlation was analyzed between the coagulation function and Child-Push grading in patients with cirrhosis. All the correlationship were assessed by t test.Results ： Significant differences of coagulation function were observed between patients with cirrhosis and controls and between patients with chronic hepatitis and controls （Pvr〈0.05、 PAPTV〈0.01, PTT〈0.05, PFIB〈0.05）. There were significant differences of coagulation between patients with Child-Push C grade cirrhosis and controls（Pall〈0.01） and between patients with Child-Push B grade cirrhosis and controls（PPT〈0.05、 PAPTT〈0.01, PTT〈0.05, PRB〈0.05）.In addition, significant differences of parameters of platelet were observed between patients with cirrhosis and controls （PPLT〈0.01, PMPV〈0.05、 PPCT〈0.01、 PPDW〈0.01 ） and between patients with chronic hepatitis and controls （PPLT〈0.058, PPCT〈0.05 ） . Conclusions ： indexs of coagulation function and parameters can be used to evaluate the degree of Liver damage, coagulation function of patients with liver disease and prognosis of liver diseases.
