优化流程管理对ICU多发伤患者伤口感染的影响研究

摘要 目的调查优化流程管理前后重症监护病房(ICU)多发外伤患者伤口感染率和感染管理质量,探讨优化流程管理对ICU多发外伤患者伤口感染的影响。方法选择2009年6月-2011年6月优化流程管理前ICU收治的102例多发外伤患者为对照组,2011年7月-2013年6月优化流程管理后ICU收治的100例多发外伤患者为观察组,比较两组患者伤口感染率以及感染管理质量的改善,采用SPSS18.0统计软件对数据进行处理。结果优化流程管理前对照组患者伤口感染率为7.8%,优化流程管理后观察组患者伤口感染率为1.0%,实施管理前后比较,差异有统计学意义(P<0.05);优化流程管理实施前环境布局、无菌技术、消毒隔离、操作规程、医疗废物处置及健康教育覆盖率各项管理质量得分,分别为(98.77±2.68)、(98.21±2.32)、(99.16±2.75)、(97.92±2.18)、(98.63±2.54)及(99.06±1.27)分,实施后分别为(89.93±3.51)、(90.37±3.12)、(92.14±4.03)、(91.07±3.01)、(91.56±2.89)及(91.31±2.37)分,差异有统计学意义(P<0.05)。结论优化流程管理的实施能够降低多发外伤患者伤口感染率,同时能够提升科室感染管理质量。 OBJECTIVE To investigate the wound infection rate and infection management quality for multiple trauma patients before and after the implementation of process management optimization in intensive care unit(ICU).METHODS Totally 102 multiple trauma patients(control group)admitted before the implementation of process management optimization from Jun.2009 to Jun.2011 were enrolled,as well as 100 multiple trauma patients(observation group)after the implementation of optimization process management from Jul.2011 to Jun.2013.The wound infection rate and management quality were compared between these two groups.Data were processed with SPSS 18.0statistical software.RESULTS The wound infection rate was 7.8%(8/102)in the control group and 1%(1/100)in the observation group,and the difference was statistically significant(P〈0.05).Scores of each item for the management quality was significantly different before and after implementation of process management optimization(98.77±2.68 vs 89.93±3.51 for environment layout,98.21±2.32 vs 90.37±3.12 for sterility technology,99.16±2.75 vs 92.14±4.03 for disinfection and isolation,97.92±2.18 vs 91.07±3.01 for operation procedure,98.63±2.54 vs 91.56±2.89 for medical waste disposal,99.06±1.27 vs 91.31±2.37 for health education coverage,respectively,P〈0.05).CONCLUSION Process management optimization can decrease the wound infection incidence in multiple trauma patients,and improve the quality of infection control.
