摘要
目的：比较不同麻醉方式对胃癌根治术后患者疼痛程度的影响.方法：选取我院于2011年3月～2013年3月收治的65例行胃癌根治术患者的临床资料,将其按照住院先后顺序分为对照组和观察组,对照组32例患者采用全麻,观察组33例患者采用全麻联合硬膜外阻滞麻醉,对比两组患者在手术结束12h后使用吗啡的剂量和在不同时间内的VAS评分,观察两组患者术后不良反应情况.结果：两组患者在完成手术12h以后,观察组的吗啡用量和重度疼痛发生率与对照组相比明显减少,比较两组差异具有统计学意义（P〈0.05）;通过观察两组患者在术后1h、4h、8h、12h和24h的VAS评分,对照组的评分明显高于观察组,比较两组差异具有统计学意义（P〈0.05）;比较两组患者的不良反应情况不具有统计学意义（P〉0.05）.结论：在行胃癌根除术后采用全麻联合硬膜外阻滞麻醉,可以有效降低患者的疼痛程度,吗啡用量明显减少,安全有效,值得临床推广应用.
Objective：To compare the influence of different anesthesia methods on pain degree of patients after radical operation forgastric cancer. Methods： Selected the clinical data of 65 patients in our hospital from March 2011 to March 2013 who had received radicaloperation for gastric cancer and divided them into control group and observation group according to the sequential order of hospitalization.32 patients in control group were given general anesthesia and 33 patients in observation group were given general anesthesia combinedwith epidural block anesthesia. 12 hours after the operation, compared the using dosage of morphine and VAS score at different time ofpatients in the two groups. Observed the postoperative adverse reactions of patients in the two groups. Results： 12 hours after the operation,compared with the control group, the using dosage of morphine and incidence of severe pain of the observation group decreasedsignificantly. The difference was statistically significant （P〈0.05）; for the VAS score respectively observed 1h, 4h, 8h, 12h and 24h after theoperation,, the scores of the control group were significantly higher than that of the observation group. The differences were statisticallysignificant （P〈0.05）; The comparison of adverse reactions of the two groups was not statistically significant （P〉0.05）. Conclusion： Generalanesthesia combined epidural block anesthesia after radical operation for gastric can effectively reduce the pain degree of patients and themorphine dosage. It is safe and effective, so it is worthy of clinical promotion and and application.
