摘要
目的 探讨脑胶质瘤患者认知功能障碍与脑功能连接网络“小世界”特性之间的关系.方法 采集20例脑胶质瘤患者和20例健康成人脑磁图数据,计算大脑静息态功能连接网络的“小世界”指数.同时进行认知功能测试,测试内容包括注意、言语流畅性、记忆力、运动能力、视觉空间能力及智力等6项测试.结果 健康受试者中,低gamma频段与注意、视觉空间、计算和相似测试结果呈正相关(P值分别为0.049,0.006,0.040,0.000);高gamma频段与视觉空间、计算和填图测试结果呈正相关(P值分别为0.04,0.040,0.023);alpha频段与相似测试结果呈正相关(P=0.040).脑胶质瘤患者在注意力、言语流畅性、计算、数字符号及填图等测试项目的Z分数显著下(P值分别为0.030,0.000,0.007,0.030,0.008),低gamma和高gamma频段“小世界”特征消失.脑胶质瘤患者中,高gamma频段与言语流畅性测试结果呈正相关(P=0.030);alpha频段与计算和填图测试结果呈正相关(P值均为0.020).结论 脑功能连接网络低gamma频段和高gamma频段“小世界”指数与认知功能存在广泛相关.脑胶质瘤患者高gamma和低gamma频段“小世界”特性消失与认知功能障碍明显相关.
Objective To investigate the brain functional connectivity network's small world' characteristics and evaluate the relationship between cognitive dysfunction and functional brain connectivity network.Methods Magnetoencephalography(MEG) recording were conducted during eyes-closed resting -state in 20 glioma patients and 20 healthy controls.The clustering coefficient,average path length and "small world" index were calculated in six frequency bands.All of them underwent 6 neuropsychological tests(including attention,verbal fluency,memory,sports ability,visual space ability and intelligence).Results In health controls,there were positive correlations between lower gamma frequency band "small world" index and attention,visual space,calculation and similar test results (P =0.049,0.006,0.040,0.000),there were positive correlations between upper gamma frequency band "small world" index and visual space,calculation and mapping test results (P =0.040,0.040,0.023).There were positive correlations between alpha frequency band "small world" index and similar test results (P =0.04).Compared to health controls,patients performed poorer Z scores in attention,verbal fluency,calculation,digit symbol and mapping tests (P =0.030,0.000,0.007,0.030,0.008),the lower gamma and upper gamma frequency band "small world" features disappeared.In glioma patients,upper gamma frequency band "small world" index and verbal fluency test results were positively correlated (P =0.030),alpha frequency band "small world" index and computation and mapping test results were positively correlated(P =0.020).Conclusions Lowed gamma frequency band and upper gamma frequency band "small world" index in functional connected network is widely correlated with cognitive functions.upper gamma and lower gamma frequency band functional connectivity network "small world" characteristic is disminished in patients with gliomas,and is significantly correlated with cognitive dysfunction.
