尺骨撞击综合征腕骨囊性变的转归及临床分析 被引量:9

The outcomes and clinical analysis of cystic changes of carpal bones in ulnar impaction syndrome

摘要 目的 研究尺骨撞击综合征中发生腕骨囊性变的相关因素及其在尺骨短缩术后的转归.方法 分析研究2008年1月至2013年6月明确诊断为尺骨撞击综合征并进行尺骨短缩手术的61例患者,从性别、年龄、手别、病程、尺骨正向变异等方面进行对比分析,总结可能引起腕骨囊性变的原因,并随访观察囊性变在X线片上的改变.结果 本组61例患者中有16例出现腕骨囊性变,发生率为26.2%.男10例,女6例;左手7例,右手9例;平均年龄47岁,平均病程11个月.实施尺骨短缩手术后随访1~25个月,3例失访,4例患者囊性变在术后0.5~2.0年消失.结论 腕骨囊性变与年龄具有显著相关性,与性别、手别、病程长短及尺骨正向变异程度无明显关联,且在尺骨短缩手术后有自愈可能. Objective To investigate the factors associated with the occurrence of cystic changes in carpal bones in ulnar impaction syndrome and treatment outcomes of ulnar shortening osteotomy.Methods Sixty-one cases of definite diagnosis of ulnar impaction syndrome treated with ulnar shortening osteotomy from January 2008 to June 2013 were reviewed.To determine the factors associated with carpal bone cystic changes,a comparative analysis was performed with respect to gender,age,side of hand involved,duration of symptoms and ulnar variance.The cystic change area was measured on radiographs taken during follow-up.Results Among the 61 patients 16 had cystic changes of the carpal bones.The occurrence rate was 26.2%.There were 10 men and 6 women with an average age of 47 years.Left hands were involved in 7 cases while right hands in 9 cases.The average duration of symptoms was 11 months.Follow-up period ranged from 1 to 25 months after ulnar shortening.Three patients were lost to follow up.In 4 patients,cystic changes had completely disappeared at the last follow-up from 6 months to 2 years.Conclusion The cystic changes were found to only correlate with age.They tend to self-heal after ulnar shortening osteotomy.
