摘要
目的探讨双倍剂量阿托伐他汀联合贝那普利对持续性房颤复律的影响。方法将86例持续性房颤患者随机分为对照组和观察组，每组43例，均采用胺碘酮或电复律进行复律。对照组在治疗前14d及治疗后采用单倍剂量阿托伐他汀联合贝那普利，观察组在治疗前14d及治疗后采用双倍剂量阿托伐他汀联合贝那普利。观察治疗后1、3、6个月时心房颤动复律后窦性心律的维持情况，服药后6个月检测左房内径，抽血测定血浆超敏C-反应蛋白（hs—CRP）和N末端脑钠肽前体（NT-proBNP）水平。结果对照组成功复律33例（76．7％），观察组成功复律39例（90．7％），两组比较差异有统计学意义（P〈0．01）。观察组在治疗后3、6个月时窦性心律的维持率均高于对照组（P〈0．05）。治疗前两组左房内径比较差异无统计学意义（P〉0．05），治疗6个月后观察组左房内径明显小于对照组（P〈0．05）。两组在治疗后均可明显降低患者的血浆hs—CRP和NT—proBNP水平，并且观察组降低程度更加明显（P〈0．05）。结论双倍剂量阿托伐他汀联合贝那普利可以明显提高持续性房颤患者的复律成功率，更好地维持窦性心律，减轻心房重构，减轻炎症反应。
Objective To study the effect of double dose of atorvastatin combined with benazepril on the cardioversion of patients with persistent atrial fibrillation. Methods Eighty-six patients with persistent atrial fibrillation were randomly divided into two groups, the control group and the observation group, with 43 cases in each group. They were given electrical cardioversion or amiodarone cardioversion. The patients in control group were treated with single dose of atorvastatin combined with benazepril 14 days before treatment and after treatment, while the patients in the observation group were given double dose of atorvastatin combined with benazepril 14 days before treatment and after treatment. One month, 3 and 6 months after treatment, the maintenance of sinus rhythm after cardioversion of atrial fibrillation was observed, after 6 months of medication, the left atrial diameter was detected, and the blood plasma high-sensitivity C-reactive protein （hs-CRP） and N-terminal brain natriuretic peptide （NT-proB- NP） levels were detected. Results Successful eardioversion was achieved in 33 cases （76.7%） of the control group and 39 cases （90.7%） of the observation group respectively, there were significant differences （P 〈0.01）. After 3 months, 6 months of treatment, the maintenance of sinus rhythm rate in the observation group was higher than that in the control group （ P 〈 0.05 ）. Before treatment, there was no significant difference in the left atrial diameter between the two groups （P 〉 0. 05 ）. After 6 months of treatment, the left atrial diameter in the observation group was significantly smaller than that in the control group （ P 〈 0. 05 ）. The plasma hs-CRP and NT-proBNP levels significantly reduced after treatment in the two groups, and those in the observation group were significantly lower （P 〈 0.05 ）. Conclusions Double dose of atorvastatin combined with benazepril can significantly increase the cardioversion success rate, and better maintain the sinus rhythm, reduce the atrial remodeling and inflammation in patients with persistent atrial fibrillation.
