摘要
目的探讨舒芬太尼对心脏瓣膜置换术患者心肌酶的影响。方法将40例择期行心脏瓣膜置换术患者随机分为观察组(舒芬太尼组)和对照组(芬太尼组)各20例。观察两组麻醉诱导前(T0)、主动脉开放即刻(T1)、主动脉开放后30 min(T2)、术毕(T3)、停机后6 h(T4)、停机后24 h(T5)时血清超敏心肌肌钙蛋白I(cTnI)和血浆肌酸磷酸激酶同工酶(CK-MB)水平。结果两组cTnI和CK-MB在T0时刻差异无统计学意义,观察组在T1~T5时点cTnI和CK-MB水平均明显低于对照组。结论舒芬太尼对心脏瓣膜置换术患者的心肌保护作用优于芬太尼。
【Objective】To compare the effects and mechanism of Sufentanil and Fentanyl on serum cTnI and CK-BM levels in patients undergoing cardiac valve replacement surgery. 【Methods】 40 anaesthetized patients with Sufentanil and Fentanyl separately in cardiac valve replacement operations were randomly divided into observation group and control group. All patients were detected the level of serum cTnI and CK-MB in different time point concluding before the induction of anesthesia(T0), in the aortic declamping moment(T1),thirty minutes after aortic declamping(T2), the end of operation(T3), 6 h(T4) and 24 h(T5) after CPB. 【Results】 There was no obviously difference in the level of cTnI and CK-MB in T0between the two groups.Compared with control group, patients in Sufentanil group showed significantly lower levels of cTnI and CKMB in T1~ T5. 【Conclusion】 Compared with equivalent doses of Fentanil, Sufentanil preconditioning could relieve myocardial injury induced by CPB in cardiac valve replacement patients.
