摘要
目的 探讨鼻内窥镜手术新的局部麻醉方式 ,以延长麻醉时间 ,提高麻醉效果。方法 在 0 .5 %地卡因反复表面麻醉下 ,对 10 4例各型、期慢性鼻窦炎、鼻息肉患者施行鼻内窥镜手术 ,观察麻醉效果 ;并对其中 3 0例无心血管系统病史、肝肾功能正常患者进行反复应用地卡因安全性的动态监测 ,且设立传统局麻对照组 ,对比研究反复表面局部麻醉的效果。结果 反复表面麻醉的总体成功率为 92 .3 % ,麻醉效果优于对照组 (P <0 .0 5 ) ;未见中枢神经系统和心血管系统的毒性反应指标出现有意义的改变 (P >0 .0 5 ) ,亦未发生因疼痛引起鼻心反射。结论 在一定时间间隔和安全用量范围内 ,使用低浓度地卡因反复表面麻醉是安全有效的 ,是慢性鼻窦炎。
Objective A new local anaesthetic method for nasal endoscopic surgery was researched to prolong anaesthetic time and to improve anaesthetic efficacy. Methods A repeated local surface anaesthesia with 0.5% Dicaine was given to 104 patients who had gone through nasal endoscopic surgery for varieties of chronic nasosinusitis and nasal polyp, and the anaesthetic efficacy was also observed. 30 in the 104 patients with normal hepatorenal function and without history of cardiovascular system disease were dynamically observed the safety of using Dicaine, and the traditional local anaesthesia was selected as control group so as to compare the efficacy of repeated surface anaesthesia. Results The total successful rate of repeated surface anaesthesia was 92.3%, which was superior to that of the control group (P<0.05); and the toxic reaction indexes in central nervous system and cardiovascular system changed insignificantly, (P>0.05); the nasal-heart reflect didn't occurr due to pain. Conclusions The repeated surface anaesthesia with low-concentration Dicaine is safe and effective, and it is a better choice in endoscopic surgery for chronic nasosinusitis and nasal polyp.
