摘要
目的 :探讨联合应用胫骨近端斜行截骨和关节镜手术治疗内侧膝关节骨关节炎的价值。方法 :对 3 7例内侧膝关节骨关节炎的患者 ,通过关节镜手术和胫骨近端斜行截骨进行治疗。术后进行 13～ 16个月的随访 ,采用Lysholm膝关节功能评分标准进行评分。结果 :胫骨近端斜行截骨操作容易 ,易于调整矫形幅度。术后 1年无矫形幅度丧失。术后 1年 ,联合应用关节镜手术和胫骨近端斜行截骨进行治疗的患者 ,Lysholm评分为 (95 .2 6± 2 .47)分 ,只做胫骨近端斜行截骨的患者为 (72 .18± 4.13 )分 ,只做关节镜手术的患者为 (65 .64± 3 .3 6)分。统计学分析表明 ,各组之间评分差异具有显著性意义 (P <0 .0 5 )。结论 :联合应用关节镜手术和胫骨近端斜行截骨治疗内侧膝关节骨关节炎是更为有效的方法 。
Objective: To study the results of oblique proximal tibial osteotomy and arthroscopy for the treatment of medial compartment gonarthrosis.Methods:Thirty seven cases of medial compartment gonarthrosis were treated through arthroscopy and oblique proximal tibial osteotomy.The plane of osteotomy runs from the anterior part of the tibia and proximal to the tibial tubercle,distally to the posterior part of the tibia,with thirty degree angulations to the tibial axis.Another eleven cases of medial gonarthrosis were treated only with oblique proximal tibial osteotomy and the other seventeen were treated only with arthroscopy.The patients were followed up thirteen to sixteen months,and evaluated according to Lysholm Knee Scale Score.The results of different methods were compared,and this way of proximal tibial osteotomy was evaluated.Results:Oblique proximal tibial osteotomy was easy to carry out,and the extent of alignment correction were easy to adjust.No loosening of correction occurred.Twelve months postoperatively,the Lysholm knee scale score were 95.26±2.47 in the group with the combined method,72.18±4.13 in the group only with oblique proximal tibial osteotomy,and 65.64±3.36 in the group treated only arthroscopically.Statistical analysis shows the difference between each two groups have prominent significance ( P<0.05 ).Conclusions:The combined method of arthroscopy and oblique proximal tibial osteotomy is a more reliable way for medial gonarthrosis.Oblique proximal tibial osteotomy is an ideal way for alignment correction.
