摘要 目的 研究视交叉池的结构特征及其手术意义。方法 在手术显微镜下观测15个头颅标本视交叉池的显微解剖。结果 视交叉池的上壁主要是视神经、视交叉及视神经间的蛛网膜,下壁为覆盖鞍膈和鞍结节的蛛网膜,池前缘上、下壁锐角交融。池内常有大量密集的纤维小梁,纵横交锗,构成纤维网复合体,可形成多种网样结构,仅50%的视交叉池与颈动脉池间存在“隔膜”。结论 视交叉池并不包绕视交叉,以致密而复杂的纤维网复合体为特征,手术中不可强行撕拉,宜在妥善保留小血管的同时小心剪开。垂体柄与视交叉的交接处常有纤维网与小动脉缠结,术中要小心。 ve To determine the microanatomic characteristics and surgical significance of chiasmatic cistern. Methods Chiasmatic cisterns were observed in 15 cadaveric human heads under an operating microscope. Results The superior wall of chiasmatic cistern mainly consisted of optic nerves , optic chiasma and the arachnoids stretching between the optic nerves. The inferior wall was a sheet of arachnoid covering diaphragma sellae and tuberculum sellae. At the anterior border, the superior and inferior walls mixed together at acute angles. There were large number of fibrous trabeculae in the cistern arranged in a crisscross pattern and they formed a fibrous network complex. This complex might consist of many sorts of network walls. There were separating membranes between the chiasmatic cistern and the carotid cistern in 50% of the specimens. Conclusion Optic chiasma is not enveloped in the chiasmatic cistern. A concentrated and complicated fibrous network complex characterizes the cistern. This complex could not be torn away by force during the operation. It should be carefully cut while the small vessels should be properly reserved. There are often small arteries tangling with the fibrous network at the area where optic chiasma is near the pituitary stalk. Procedures in this area should be careful.
