摘要
目的 在颅脑手术中通过观察尼莫地平对血浆和脑脊液中 β 内啡肽 (β EP)的影响 ,探讨尼莫地平的脑保护作用。方法 36例ASAⅠ~Ⅱ级择期颅脑手术病人 ,随机分为尼莫地平组 (Ⅰ组 )和对照组 (Ⅱ组 )。采用放射免疫分析法检测血浆及脑脊液中 β EP的含量。 结果 Ⅰ组在给药后 1小时和 2小时血浆 β EP含量分别为 (1 69 90± 80 0 0 ) pg/ml和 (74 76± 38 90 ) pg/ml,脑脊液中含量分别为 (2 2 2 6 0 7± 42 2 2 2 ) pg/ml和(1 2 85 60± 40 5 2 5) pg/ml,均较给药前 (即开始切硬膜时 )和Ⅱ组低 (P <0 0 5)。结论 颅脑手术围术期应用尼莫地平可以使脑脊液和血浆中 β EP含量降低 ,提示应用尼莫地平可以减轻脑水肿和损伤 。
Objective To observe the effect of nimodipine on plasma and cerebrospinal fluid β endorphin (β EP) during craniotomy.Methods Thirty six patients(ASAⅠ Ⅱ)undergoing neurosurgery were divided randomly into nimodipine group(group Ⅰ)and control group(group Ⅱ).β EP in plasma and cerebrospinal fluid (CSF) was measured with radioimmunoassay and MAP,HR,T monitored.Results The levels of β EP were (169 90±80 00) and (74 76±38 90)pg/ml in plasma,and (2*!226 07 ±422 22) and 1*!285 60 ±405 25)pg/ml in CSF at 1 and 2h after nimodipine respectively in group Ⅰ,which were lower significantly ( P< 0 05) compared with those before nimodipine and in group Ⅱ.Conclusion Nimodipine administration could decrease β EP in both plasma and CSF and protect the cerebral cells from edema and injury in neurosurgical patients.
