摘要
目的 观察血液流变学主要指标及卡松方程参数在常见老年病诊断治疗中的临床应用价值。方法 分别测定 197例常见老年病患者 (糖尿病 4 4例 ,冠心病 5 9例 ,高血压 31例 ,高血脂 11例 ,老慢支 30例 ,脑梗 2 3例 )和 5 4例中老年正常健康体检者的血流变指标 ,并按疾病分组进行统计分析。结果 与对照组相比 ,各疾病组 (慢支组除外 )血流变指标均有不同程度的显著增高 (P <0 .0 5 ) ,尤其全血低切表观粘度 (2S-1)、全血高切表观粘度 (2 0 0S-1)、血沉方程K值、卡松屈服应力τc 增高最普遍 ,其指标阳性率明显高于正常对照组 (P <0 .0 1)。各疾病组改变无特异性 ,但侧重点略有不同。卡松屈服应力与全血低切粘度 (2S-1)、红细胞聚集指数EAI及血沉方程K值 ,卡松粘度与全血高切粘度 (2 0 0S-1)间具有较显著的相关性 (P <0 .0 5~ 0 .0 1)。结论 血流变指标异常改变对临床治疗高粘、高聚、高凝综合症有一定参考指导意义。卡松屈服应力τc 和卡松粘度 ηc 指标的检测 ,有助于临床血流变检测结果间的统一性、可比性及检测标准化。
Objective To study the clinical significance of Hemorheology examination parameters and Casson equation index in diagnosis and therapy of common geriatric disease.Methods The measurements of viscosity and some homorheological dependent index was performed in 197 patients with common geriatric disease(diabetes mellitus44 coronary heart disease59,hypertension disease31,hyperlipemia 11,chronic obstructive lung 30 and cerebral infarction 23) and 54 healthy middle age and elderly as a control.Results There are significant differences between each disease group(except group of chronic obstructive lung) and the control(P<0.05),especially the whole blood viscosity at 2S -1 ,200S -1 shear rate,erycarocyte sedimentation rate,equation K value and Casson yield stress were significantly increased and had a higher positive rate than the control(P<0.01).The correlations between Casson field stress(τ c) and η b(2S -1 ),Casson Viscosity(η c)and η b(200S -1 )were significant at the 0.01 level.Conclusion Abnormal changes of hemorheological characteristics in patients with common geriatric disease are very common and there is no specielity,but there are slight differences in different disease groups and it may be as a reference for the therapy.The two index of Casson equation may be as an extend index of hemorheological parameters and it may be helpful for comparison and standaredization among results of different hospitals.
