摘要
目的:了解组织多普勒成像(DTI)技术在评估冠心病(CAD)心肌舒缩功能方面的应用。方法:应用超声心动图DTI技术测量正常人和冠心病患者心尖二腔观二尖瓣环、左室前壁及下壁心肌的基段、中段及心尖段收缩期、舒张早、晚期峰值运动速度(分别为S、E、A)、内外膜峰值度差(ΔV)及峰值速度阶差(MVG)、E/A比值、等容收缩期(IVC)及等容舒张期心肌运动频谱持续时间(IVR)等一系列参数,并进行比较。结果:冠心病组二尖瓣环、左室前壁及下壁收缩期、舒张早期各测量参数均低于正常,舒张晚期无明显变化,E/A比值减小,ΔV及MVG减小,IVC、IVR明显延长(P<0.05,P<0.01)。结论:DTI技术可实时、准确地反映心肌舒缩运动状况,为进一步了解局部及整体心肌功能提供更多信息。
Objective:TO assess the contractile and relaxant function of myocardium by Doppler tissue imaging (DTI) in patients with coronary artery disease(CAD). Methods:Certain parameters about mitral annulus and wall motion (anterior and interior wall of LV) from apical two-chamber view such as systolic peak velocity,early-diastolic and late-diastolic peak velocity(S、E、A respectively),velocity difference(ΔV) and velocity gradient(MVG) between endocardium and epicardium,E/A ratio,isovolumic contraction and isovolumic relaxation duration (IVC,IVR respectively) were evaluated by means of Doppler tissue imaging in 32 patients with CAD and 32 normal subjects. Results:Compared with control group,most of the systolic and diastolic motion parameters about mitral annulus and walls in CAD group were lower except for late-diastolic velocity;E/A ratio,ΔV and MVG were respectively reduced and IVC,IVR longer(P< 0.05,P< 0.01). Conclusion:Doppler tissue imaging is a useful technique that can accurately evaluate contractile and relaxant performance of global and regional myocardium in real time,and then,provide further information for assessment of global and regional myocardium function in patients with CAD.
