右室组织多普勒Tei指数评价室间隔缺损右室功能的研究 被引量:2

摘要 目的探讨组织多普勒(TDI)Tei指数评价室间隔缺损患者右心室功能的价值。方法根据患者肺动脉收缩压(PASP)水平,将56例室间隔缺损患者分为合并肺动脉高压[PH(+)]组(33例,PASP≥40 mmHg)和未合并肺动脉高压[PH(-)]组(23例,PASP<40 mmHg);选取25例健康体检者为正常对照组。利用超声TDI技术获得三尖瓣环组织运动频谱,计算研究对象的右室Tei指数。结果 PH(+)组Tei指数明显高于对照组及PH(-)组(P<0.05);PH(+)组IVRT明显大于PH(-)组(P<0.05);与对照组比较,PH(-)组IVRT、IVCT、ET及右室Tei指数均无显著差异(P>0.05)。TDI测得右室Tei指数与PASP呈正相关(P<0.01)。结论室间隔缺损合并肺高压患者右室整体功能受损,TDI-Tei指数可较准确反映室间隔缺损患者右室功能。 Objective To investigate the value of tissue Doppler imaging (TDI)Tei index in the evaluation of the right ventricular(RV) function of patients with ventricular septal defect (VSD). Methods Fifty six patients with VSD were divided into group with complication of pulmonary hypertension [ PH ( + ) group ] ( PASP ≥ 40 mmHg, n = 33 ) and group without pulmonary hypertension [ PH ( - ) group ] ( PASP 〈 40 mmHg, n = 23 ) according to the levels of their pulmonary arterial systdic pressure (PASP). And 25 normal individuals were enrolled as control group. The movement frequency spectrum of tricuspid annulus tissues were obtained by ultrasound TDI, and the subjects' RV Tei indexes were measured. Results The RV Tei index of PH ( + )group was significantly higher than that of PH ( - ) group and the control group ( P 〈 0.05 ). IVRT values of PH ( + ) group were higher than those of PH ( - ) group(P 〈 0.05). There was no significant difference in IVRT, IVCT, and ET, and RV Tei indexes between PH ( - )group and the control group( P 〉 0.05 ). TDI detection results showed there was a positive correlation between RV Tei index and PASP( P 〈 0.01 ). Conclusion The right ventricular total function of patients with VSD complicated with pulmonary hypertension is damaged. TDI Tei index can exactly reflect their right ventricular function.
