摘要
目的 :探讨四肢骨折合并多发损伤的治疗及疗效。方法 :自 1997～ 2 0 0 1年共收治合并其他脏器损伤的四肢骨折 118例 ,骨折早期 (2 4h内 )内固定 6 0例 ,延迟 (2 4h后 )内固定或非手术治疗 5 8例。结果 :前者术后并发症发生率低于后者 ,平均住院日明显缩短 ,骨折畸形愈合率显著降低 ,对 4 0例相同脏器损伤ISS评分接近的严重损伤患者 ,2 4h内行骨折内固定 15例 ,死亡 1例 (7% ) ,2 4h后行骨折内固定或非手术治疗 2 5例 ,死亡11例 (44 % ) ,P <0 .0 5。结论 :对合并其他脏器损伤的患者早期 (2 4h内 )骨折内固定可降低死亡率 ,减少严重并发症 ;促进肢体及脏器功能的恢复。
Objective:To discuss the treatment of the limb fracture complicated with multiple injuries and its effect.Methods: One hundred and eighteen cases with limb fracture complicated with organic injuries were treated from 1997 to 2001. Among them sixty cases had early(24h)internal fixation, other fifty-eight eases had late internal fixation or non-operation.Results:The incidence of complications after operation in the former was much lower than in the latter and so was the average hospital stay or malunion.Among the fourty cases with serious analogous organic injuries with approximate Iss complicated with limb fracture,fifteen cases had early (24h)internal fixation and 1 case (7%)died, other 25 eases had late operation or non-operation and 12 cases(44%)died, P <0.05. Conclusions:Early (24h)internal fixation to the limb fracture of patients with organic injury can decline mortality,prevent serious complication and promote the recovery of limb and organic function.
