摘要 目的探讨MRI诊断脑转移瘤的价值。方法分析36例脑转移瘤患者的临床及MRI资料,全部病例均行非增强及增强MRI检查。结果多发颅脑转移瘤29例,其中3例为弥漫性脑转移瘤。单发脑转移瘤7例。脑转移瘤发生在幕上者21例,幕下者4例,幕上及幕下均有者11例。4例伴有邻近脑膜受累。非增强MRI病灶上多数为长T1长T2信号,增强MRI上病灶均有强化,以环状强化为多见。增强MRI比非增强MRI发现转移瘤病灶将近高1倍。结论增强扫描是MRI诊断脑转移瘤所必须的。结合临床病史和MRI特征,对大多数病例可做出准确诊断。 Objective To evaluate the diagnostic value of MRI in cerebral metastasis.Methods Thirty-six patients with cerebral metastasis underwent MR imaging without and with contrast enhancement and then clinical and MRI findings in all patients were analyzed.Results Among 36 cases,7cases were single metastasis and 29 cases were multiple metastasis in which 3 cases were diffuse metastasis,The lesions sited on superior tentorium were found in 21 cases,inferior tentorium in 4 cases,and both superior and inferior tentorium in 11 cases.The lesions involved into meninges were found in 4 cases.Most lesions showed a long T1 and long T2 signal intensity on non-contrast enhancement MRI and the circular-enhanced feature on contrast enhancement MRI.The number of lesions detected on the enhancement MRI were higher approximately one time than that on the non-contrast enhancement MRI.Conclusion The enhancement scan of MRI is necessary to diagnose cerebral metastasis.Most cases of cerebral metastasis can be diagnosed according to the MRI features combined with history.
