摘要
为了探讨白介素-4(Interleukin-4,IL-4)和白介素-6(Interleukin-6,IL-6)在慢性阻塞性肺病(COPD)患者肺组织中的表达变化及其在COPD发病机制中的作用,45例因肺癌而行肺叶切除术患者的肺组织,根据有无COPD及吸烟史分为COPD组、吸烟对照组及非吸烟对照组(每组各15例).用免疫组织化学法测定肺组织炎性细胞中IL-4和IL-6的表达.结果表明:1.IL-4和IL-6在COPD组、吸烟对照组及非吸烟对照组患者肺泡局部浸润的炎性细胞中有表达,其中COPD组IL-4和IL-6的表达强度明显高于吸烟对照组及非吸烟对照组(P<0.05),但吸烟对照组及非吸烟对照组IL-4和IL-6表达强度无显著差别(P>0.05).2.三组中IL-4在肺组织中的表达强度与1秒钟用力呼气容积占预计值百分比(FEV1%pred)值之间均无直线相关关系(r=-0.093,0.00245和-0.252,P>0.05);仅在COPD组肺组织中的IL-6表达强度与FEV1%p re值呈直线负相关关系(r=-0.63119,P<0.05),其余各组未发现这种负相关关系.3.COPD组IL-4与IL-6的表达呈直线正相关关系(r=0.599,P<0.01).结果提示:1.IL-4在COPD患者细支气管及肺泡炎性细胞中表达增强,说明Th2型细胞因子可能也在CO PD发病过程发挥重要作用;2.IL-6在COPD患者肺组织炎性细胞中表达增强与COPD气道阻塞密切相关,这可能与其在局部加重炎症和气道损伤有关.
In order to investigate the changes of interleukin 4 and interleukin 6 expression in chronic obstructive pulmonary disease(COPD)and their effect on the pathogenesis of COPD,lung specimens were obtained from 45 lung cancer patients by pneumectomy The 45 cases were divided into a COPD group,a smoking control group and a nonsmoking control group,according to the COPD diagnosis and smoking history Immunohistochemical method was applied to identify the expression of IL 4 and IL 6 in inflammatory cells of pulmonay tissues The result showed that:1)IL 4 and IL 6 were expressed in the local in filtrated in flammatary cells in bronchioli and alveoli,and the expression of IL 4 and IL 6 higher in the COPD group was markedly than in the smoking and nonsmoking control groups(P<005),but there was no distinct difference between the later two groups(P>005)2)No correlation in IL 4 and FEV1%was found in any of the groups negative correlation between IL 6 and FEV 1%pre was found in the COPD group(r=-063119,P<005),but not in other groups 3)The expression of IL 4 was positively correlated with the expression of IL 6(r=0599,P<001)in the COPD group The study indicates that:1)the high expression of IL 4 in COPD suggested Th 2 type cytokine might also play an important role in the pathogenesis of COPD;and 2)The high expression of IL 6 might play an important role in the progress of airway obstruction,which might reflect,and aggravate the local inflammation.
