摘要
目的 :探讨肾上腺外伤性血肿的CT表现及其诊断价值。材料和方法 :7例车祸伤后肾上腺血肿患者 ,其中 1例亚急性期方发现病灶 ,分别于急性期、亚急性期、慢性期行CT扫描。结果 :急性期 ,6例平扫呈均匀稍高密度灶 ,增强扫描无增强 ,其中 4例见残存正常肾上腺增强明显 ;亚急性期 ,本期方发现病灶 1例外 ,余急性期 6例血肿均有不同程度缩小 ,全部 7例血肿密度不均 ,呈环状增强 ;慢性期 ,3例血肿显著缩小机化呈均匀明显增强结节 ,3例血肿吸收消失 ,1例囊变。结论 :肾上腺外伤性血肿各期CT表现有一定特征 ,CT动态扫描可对其诊断与鉴别诊断提供可靠依据 。
purpose: To study the CT manifestations and its diagnostic value in the adrenal injurious hematomas. Materials and Methods: 7 cases with adrenal hematomas were injured in car accidents and CT scan were performed all cases at acute,subacute and chronic stage. Results: Of 7 lesions, during the acute stage 6 were high homogeneous density by plain scaned and no enhancement on CT examination, 4 of 6 normal parts of adrenal showed marked enhancement. During the subacute stage, 6 were reduced areas except the one which the hematoma was found in this stage,7 were ring like enhancement and enhanced inhomogeneousty. During the chronic stage, CT features included: ①complete disappearance(n=3), ②significant reduction and organization,with homogeneousty enhanced nodes(n=3), ③cysted lesion(n=1). Conclusion: CT features of adrenal injurious hematoma have some characteristics. The results seggest that knowledge of the CT manifestation of adrenal injurious hematomas is useful for the diagnosis and management of the patients.
