摘要 目的 探讨乳腺癌原发灶CD44V6的表达与淋巴结微转移及CD44V6,CEA和CK19在检测淋巴结微转移中的作用。方法 采用SP免疫组化染色法检测 10 5例乳腺癌原发灶CD44V6的表达及腋下淋巴结 (转移阳性 5 4例 ,阴性 5 1例 )CD44V6,CEA和CK19阳性表达检出率。结果 淋巴结转移阳性 5 4例 ,CD44V6,CEA和CK19阳性表达百分率分别为 85 2 % ,90 7%和 10 0 %。淋巴结阴性 5 1例 ,有CD44V6,CEA和CK19阳性表达百分率分别为 15 7% ,9 8%和15 7% ,三者共在淋巴结转移阴性 5 1例中检出有微转移病灶存在 13例。原发灶CD44V6阳性表达百分率为 74 3 % ,与腋下淋巴结转移有关 (χ2 =8 3 8,P <0 0 0 5 ) ,淋巴结转移灶CD44V6阳性表达百分率为 89 6%。结论 乳腺癌腋下淋巴结常规检查阴性病例 ,若原发灶表达CD44V6阳性 ,则行淋巴结CD44V6和CK19免疫组化检测 。 Objective To investigate the expression of CD44V6 in breast cancer and micro-metastasis of lymph node and the role of CD44V6, CEA and CK19 in detecting lymph node micro-metastasis. Methods The expression of CD44V6 in breast cancer was detected by immunohistochemistry in 105 specimen of breast cancer. The expression of CD44V6, CEA and CK19 was found in 54 positive metastasis of axillary lymph node and 51 lymph nodes without metastasis. Results Positive rate of CD44V6, CEA and CK19 in metastatic lymph node was 85.2% , 90.7% and 100% respectively. The positive rate of CD44V6, CEA and CK19 in negative lymph node was 15.7% , 9.8% and 15.7% respectively. 13 out of 51 cases of negative lymph node had micro-metastasis. The positive rate of CD44V6 in breast cancer was 74.3% , associated with metastasis of lymph node. The positive rate of CD44V6 in metastatic lymph node was 89 6%. Conclusion In axillary lymph node of breast cancer without signs of metastasis on normal examination, if expression of CD44V6 in breast cancer is positive, detection of CD44V6, CK19 in axillary lymph node by immunohistochemistry may help the diagnosis of micro-metastasis of axillary lymph node.
