摘要
目的 :探讨原发性高血压 (EH)心功能不全患者血清尿酸 (UA)水平的变化及氯沙坦 (Los)对其影响。方法 :比较EH60例不同心功能状态下尿酸浓度 ,同时将60例患者分为Los组和Enalapril(Ena)组两组 ,每组30例 ,Los组给50Los50mg1日1次 ;Ena组Ena10mg1日1次。疗程4周 ,观察治疗前后UA变化。结果 :(1)EH心功能I、Ⅱ、Ⅲ、Ⅳ级的UA分别为 (4 8±1 4) ,(5 2±1 2) ,(7 5±2 0) ,(9 2±2 3)mg/dl,心功能Ⅳ级者明显高于Ⅲ级患者 (P<0 01)。 (2)Los组治疗前后UA分别为(7 6±2 4) ,(5 5±1 9)mg/dl(P<0 01) ,Ena组治疗前后分别为 (7 3±3 1) ,(6 2±2 0)mg/dl(P<0 05)。两组治疗后比较有显著差异 (P<0 05 ,t=2 08)。结论 :EH心功能不全患者UA随着心功能不全加重而明显升高。Los在改善心功能同时 ,还具有降尿酸作用。
Objective: To study the influenec of Losartan on variation of serum uric acid in patients with cardiac dysfunction of primary hypertension.Methods: The concentrations of serum uric acid in 60 patients of primary hypertension in distinct state of cardiac funtion were compared,meanwhile the 60 patients were divided into Losartan and Enalapril groups,each group 30cases.In the Losartan group was giren Losartan 50mg orally once a day,in the other group was giren Enalapril lomg orally once a dqy,both the treatment time of the two groups were 4 weeks.The variations of serum uric acid wrer observed before and after rteatment respectively.Results:The concentration of serum uric acid in patients with primary hypertension in cardiac function Class I?Ⅱ?Ⅲ?Ⅳ was (4 8±1 4),(5 2±1 2),(7 5±2 0) and (9 2±2 3)mg/dl respectively,and the concentration of Serum uric acid in class Ⅳ was much higher than that in ClassⅢ (p<0 01).The concentration of serum uric acid in Losartan group was (7 6±2 4) and (5 5±1 9) mg/dl before and after the treatment respectively (p<0.01),while in Enalapril group was (7.3±3.1) and (6.2±20)mg/dl respectioely (p<0.05),and there was significant difference between the two groups after treatment (p<0.05,t=2.08).Conclusion: The concentration of serum uric acid in patients with cardiac dysfunction of primary hypertension is obviouly rising along with the condition worsened of cardiac dysfunction.Losartan will improve cardiac function and has a role to decrease the serum uric acid ,it also gains most effective protecting for the benefit of cardiovascular system.
