摘要
探讨围绝经期抑郁发作患者甲状腺激素水平的变化情况。方法将99例围绝经期抑郁发作患者设为研究组,40例女性围绝经期健康者设为对照组。分别于治疗前后采用汉密顿焦虑量表、汉密顿抑郁量表评定两组患者的焦虑、抑郁程度,测定两组血清甲状腺激素水平。结果治疗前研究组血清总三碘甲状原氨酸、游离三碘甲状腺原氨酸及游离甲状腺素水平均显著高于对照组(P<0.05),促甲状腺激素水平显著低于对照组(P<0.05)。治疗4周末及8周末研究组游离甲状腺素水平较治疗前显著持续降低(P<0.01)。治疗4周末起研究组汉密顿焦虑量表及汉密顿抑郁量表评分均较治疗前显著持续降低(P<0.01),治疗8周末汉密顿抑郁量表评分显著低于治疗4周末。治疗前与治疗8周末游离甲状腺素水平变化与汉密顿焦虑量表评分变化呈显著正相关(P<0.01)。结论围绝经期抑郁发作患者甲状腺激素水平存在异常,血清游离甲状腺素水平与患者焦虑抑郁水平相关。
Objective To investigate the changes of thyroid hormone levels in patients with perimenopaus-al depressive episodes.Methods A total of 99 patients with perimenopausal depressive episodes were set as the study group ,and 40 healthy perimenopausal women were set as the control group.Before and after treatment ,the Hamilton anxiety scale and the Hamilton depression scale were used to evaluate the anxiety and depression of the two groups ,and the serum thyroid hormone levels were measured.Results The levels of TT3 ,FT3 and FT4 in the study group were significantly higher than those in the control group before treatment (P< 0.05) ,and the TSH level was significantly lower than that in the control group (P<0.05).At the end of the 4th and 8th weeks of treatment ,the FT4 level of the study group was sig-nificantly lower than that before treatment (P<0.01).At the end of the 4th week of treatment ,the HA-MA and HAMD scores of the study group were significantly lower than those before treatment (P<0.01) ,and the HAMD score at the 8th week of treatment was significantly lower than the 4th week of treatment.The difference between FT4 and HAMA scores was statistically significant before treatment and 8 weeks after treatment (P<0.01).Conclusion There is abnormal thyroid hormone level in patients with perimenopausal depressive episodes ,and serum FT4 level is associated with anxiety and depression levels.
