摘要
目的 评价对比剂不同注射剂量和流率对胰腺CT增强的影响。资料与方法 5 0例患者按不同剂量和注射流率随机分成 5组 ,Ⅰ组 (1.5ml/kg、3ml/s) ,Ⅱ组 (1.5ml/kg、2 .5ml/s) ,Ⅲ组 (1ml/kg、3ml/s) ,Ⅳ组 (1ml/kg、2 .5ml/s) ,Ⅴ组 (固定剂量 75ml、2 .5ml/s) ,每组各 10例 ,分别行胰腺薄层双期扫描 ,实质期和门脉期的延迟时间分别为 40s、70s。分别测量平均胰腺CT增强值并作比较。结果 实质期平均胰腺CT增强值 ,Ⅰ组为 71.0 5± 9.6 4HU ,Ⅱ组为 6 4.0 4± 9.6 1HU ,Ⅲ组为 45 .16± 7.98HU ,Ⅳ组为 44 .16± 11.2 7HU ,Ⅴ组为 5 0 .2 0± 13.5 9HU。Ⅰ与Ⅲ组相比(P <0 .0 5 ) ,Ⅱ组与Ⅳ组相比 (P <0 .0 5 ) ,Ⅱ组与Ⅴ组相比 (P <0 .0 5 )有明显差异。Ⅰ与Ⅱ、Ⅲ与Ⅳ、Ⅳ与Ⅴ组相比无明显差异。结论 高剂量可提高胰腺增强效果 ,1.5ml/kg注射剂量 ,2 .5~ 3ml/s注射流率是比较合适的注射参数。
Objective To assess the effects of different intravenous injection doses and rates of contrast material on pancreatic enhancement in CT scanning.Materials and Methods Fifty patients were randomly and equally divided into 5 groups. The injection dose and rate of the contrast medium in each group was as follows. Group Ⅰ: 1.5ml/kg, 3ml/s; Group Ⅱ: 1.5ml/kg, 2.5ml/s; Group Ⅲ: 1ml/kg, 3ml/s; Group Ⅳ: 1ml/kg, 2.5ml/s; and Group Ⅴ: fixed dose of 75ml, 2.5ml/s. Dual phase and thin slice scanning of the pancreas was performed in all cases. The delayed time for parenchymal and portal phase scanning was 40s and 70s, respectively. Mean CT enhancement value of pancreas in each group was calculated respectively. Results Mean CT enhancement value of pancreas in parenchymal phase in each group was as follows. Group Ⅰ, 71.05±9.64 Hu; Group Ⅱ, 64.04±9.61Hu; Group Ⅲ, 45.16(7.98Hu; Group Ⅳ, 44.16±11.27Hu; and Group Ⅴ, 50.20±13.59Hu. Significant difference in mean pancreatic enhancement value was found between Group Ⅰ and Ⅲ, between Group Ⅱ and Ⅳ, and between Group Ⅱ and Ⅴ (P<0.05). No significant existed between Group Ⅰ and Ⅱ, between Group Ⅲ and Ⅳ, and between Group Ⅳ and Ⅴ.Conclusion High dose of contrast medium can increase the enhancement value of the pancreas. A dose of 1.5ml/kg with an injection rate of 2.5~3ml/s is recommended.
