摘要
目的观察L-精氨酸联用牛磺酸试治充血性心衰的疗效作用。方法将41例慢性充血性心衰(CHF)病人随机分为治疗组和对照组。在一般抗心衰药物基础上治疗组L-精氨酸150mg/kg体重和牛磺酸200mg/kg体重溶于250ml生理盐水静脉滴注,qd,6天为1疗程。对观察前后心功能指标、生活质量、血小板cGMP、血浆N-末端心钠素(N-ANP)、血浆内皮素(PET)、去甲肾上腺素(PNE)、肾素活性(PRA)、尿cGMP和NO_2^-/NO_3^-等进行显著性检验。结果治疗组显效率92.68%(38/41),心功能指标和生活质量明显改善、血小板cGMP显著增加(P<0.01);PET显著降低(P<0.05);尿cGMP和NO_2^-/NO_3^-显著增加,(P<0.05,P<0.01)。结论 L-精氨酸联用牛磺酸治疗CHF可以提高病人对一般抗心衰药物得敏感性和药效作用,改善心功能和血管顺应性,提供内源性NO以弥补NO缺陷,并保护心肌细胞,从而显示出有益的治疗作用。
Objective To observate the effectiveness of treating patients with chionic congestive heart failure (CHF) in using Larginine and taurine. Methods To divice randomized patients with CHF into therapy group and control group. In the basis of general durgs of treating CHF, L - arginine 150mg/kg and 200mg/kg into 250ml normal saline are dropped in veine passway, it is treated the patients of the therapy group in short period, qd. for 6days as 1 course. Before and after duration, index of heart function, living quality are detected and cGMP levels of platelet, the levels of plasma N - ANP en-dothelin (PET), norepinephrine (PNE), renal activity (PRA) and N-ANP are detected by radioimmunoassay. The data of the two groups are compared by significant difference. Results Therapy group achieved 92.68 %, index of heart function and living quality were improved, cGMG level of platelet is increased (P < 0.01); PET level is dropped (P < 0.01), cGMP and NO2- /NO3- of urine obvious are enhanceed, (P<0.05, P<0.01) . Other datd were not found significant difference between therapy group and control group. Conclusion: L- arginine with taurine therapy enhances the patients with CHF to the sensitivity and effectiveness to general drugs of treating CHF, and it improves vasocular compliance and provides endogenous nitric oxide to remedy its dificiency to protect myocardial cells. It displays beneficial effect for the patients with CHF.
