摘要
目的 :了解心理干预对原发性闭角型青光眼手术患者心理状况的影响 ,建立适合于青光眼患者的围术期心理干预措施。方法 :随机抽取原发性闭角型青光眼手术患者 80例 ,其中干预组 40例 ,对照组40例。采用支持性心理治疗、松弛训练和自我催眠疗法对干预组患者进行术前、术中、术后心理干预。结果 :与对照组比干预组患者术前 1日和术后 3日的状态焦虑、抑郁评分及术前 1日和术后当日的心率、收缩压明显减低 ,术中疼痛不适程度较轻 ,手术满意度较高。讨论 :支持性心理治疗、松弛训练和自我催眠疗法是青光眼患者围术期心理干预的有效措施 ,便于临床实施 ,值得推广应用。
Objective:To evaluate the effect of psychological inte rvention on mental state of surgical patients with primary angle-closure glaucom a.Mehod:80 patients with primary angle-closure glaucoma waiting for operation were randomly divided into intervention group(40 cases)and control gro up(40 cases).The intervention group received supportive psychotherapy,self-rela x ation training,self-hypnosis training before operation,and practiced the learnt skills during and after operation.Both groups were evaluated with EPQ,SSRS(soci al support rating scale),STAI(state-trait anxiety inventory),SDS(self-rating depre ssion scale),VAS-pain(visual analogue scale),their heart rate and blood pressure were measured before,during and after operation.Results:Compared w it h control,the intervention group had lower scores on state anxiety,depression 1 day before operation and 3 days after operation.They had lower heart rate and B P during operation,had less pain and were more satisfied with the medical service .Conclusion:Supportive psychotherapy,relaxation training and self-h ypnosis are effective in helping patients adapting to operation on glaucoma.
