椎板减压钛网椎管成形治疗腰椎管狭窄症 被引量:11

Treatment for the spinal stenosis with titanium rete vertebral canals reconstruction and bone allograft after laminectomy

摘要 目的 :介绍一种椎板减压后椎管成形、后路脊柱融合的方法 ,评价其在治疗腰椎管狭窄症中应用的效果。方法 :33例腰椎管狭窄症患者在行常规椎管减压解除神经根及硬膜囊压迫后 ,应用钛网行椎管成形并脊柱后路融合。分别于术前、术后 1年及术后 2年进行下腰痛Greenough和Fraser评分。 结果 :33例患者切口均一期愈合。术后 3个月复查 ,钛网位置无变化 ,已经被再生的自体骨固定。术后 1年时下腰痛评分均较术前有显著性差异 ,术后 2年时 ,评分与术后 1年时无显著性差异。结论 :腰椎管狭窄症患者在椎板减压术后应用钛网椎管成形植骨融合有利于保持治疗效果 ,避免下腰痛复发。钛网应用操作简单 ,适合临床应用。 Objective:To introduce a method to treat spinal stenosis using titanium rete and bone allograft after laminectomy,and evaluate its clinical effect.Method:Thirty three patients with spinal stenosis were treated by spinal canal decompression through laminectomy and posterior spinal fusion was performed by th way of titanium rete vertebral canal reconstruction and bone allograft.The follow up time was 2 years.We observed the clinical effect with Greenough and Fraser low back pain score.Result:All cases were primary healing;the preoperative symptoms were improved satisfactorily.3 months after operation,the allograft and autograft were confluent,there were no vertebral canal and nerve root or dura mater compression.2 years after operation,the Greenough and Fraser low back pain score was higher than that of preoperation.3 months after operation,the spinal fusion was achieved,the titanium rete was fixed biologically.The Greenough and Fraser low back pain score of 1 and 2 years after operation was increased significantly than that of preoperation.Conclusion:After titanium rete vertebral canal reconstrution,it′ll be can do laminectomy and posterior spinal fusion at the same time that insure the effect of the treatment of spinal stenosis.The technique of titanium rete vertebral canal reconstrusion and bone allograft restore the posterior construction,and its cheaper price,simpler technique,wider indication are more suitable for clinics than others.
