摘要
目的 观察麻醉剂量下丙泊酚对内隐记忆及其脑电双频谱指数 (BIS)的影响。方法 选择 4 2例子宫全切术的病人 ,随机分为三组。Ⅰ组 ,Ⅱ组为试验组 ,各 11例。Ⅲ组为对照组 2 0例。三组病人均施予腰麻硬膜外联合麻醉。试验组加用丙泊酚 2mg·kg-1,维库溴铵 8mg诱导行气管内插管 ,静脉持续泵入丙泊酚 110 μg·kg-1·min-1维持麻醉。采用HXD 1型多功能脑电监测仪监测BIS。BIS >6 0的病人为I组 ,BIS <6 0的病人为Ⅱ组 ,关腹后给予内隐记忆听刺激。术后 6h测试其模糊辨听率 ,同时对术中的记忆情况进行调查。结果 Ⅰ、Ⅱ组间脑电BIS有显著差异 ,分别为 71 93± 10 5 1和 5 6 0 4± 11 86 (P <0 0 5 )。模糊辨听率三组间无差异 (P >0 0 5 )。试验组病人无一例存在有外显记忆。结论 临床剂量的丙泊酚无论BIS大于或小于 6 0 。
Objective To assess the effect of propofol anesthesia on implicit memory and bispectral index (BIS) Methods Forty two ASA Ⅰ Ⅱ patients [mean age (46 36±6 34)yr,mean weight (64 48±9 69)kg,mean height (161 05±4 77cm)] undergoing total hysterectomy were studied Patients with hearing or memory disturbances or mental disorder were excluded The patients were divided into three groups: control group (group C, n=20) and experimental group I(group E Ⅰ, n=11) and Ⅱ(group E Ⅱ, n=11) The patients were given scopolamine 0 3mg IV in the operating room before combined spinal epidural anesthesia (CSE) The height of block was controlled below T 8 In group C operation was performed under CSE without general anesthesia combined In group E Ⅰ and E Ⅱ CSE was combined with general anesthesia Anesthesia was induced with propofol 2mg·kg -1 and vecuronium 8mg The patients were then intubated and mechanically ventilated and P ET CO 2 was controlled at 35mm Hg Anesthesia was maintained with propofol infusion at a rate of about 110μg·kg -1 ·min -1 Besides BP,ECG and SpO 2,quantitative EEG BIS and SEF and heart rate variation (HRV) were monitored during operation In group E Ⅰ BIS was above 60 and in group E Ⅱ below 60, after the last peritoneal stitch the patients were presented with stimulus words Recall of intraoperative events and muddy identified hearing rate were investigated 6h after operation Results There was no significant difference among the three groups in age, weight and height There was significant difference in BIS between group E Ⅰ (71 39±10 51) and group E Ⅱ(56 04±11 86) but there was no difference in the muddy identified hearing rate among the three groups(P>0 05) There was no explicit memory in the experimental groups Conclusions During propofol anesthesia whether BIS is >60 or <60, the implicit memory can not be inhibited
