摘要
为改革医院几十年的传统助产模式 ,建立一套全新的产房责任制助产模式 ,以保证分娩过程中母婴安全 ,提高医疗质量 ,产房实行“全程专责陪伴分娩”助产模式 ,并与传统的产房助产模式进行比较 ,观察两种模式对产房工作质量的影响。结果 :“全程专责陪伴分娩”模式实施后 ,异常阴道分娩 (吸引术、产钳术 )率下降 ,潜伏期延长、活跃期停滞、第二产程延长及滞产发生率均为 0 ,新生儿重度、轻度窒息 ,新生儿气管插管发生率明显下降 ,宫颈裂伤、会阴Ⅲ度裂伤、产后出血、会阴伤口感染率与传统模式相比 ,均有统计学意义。产房“全程专责陪伴分娩”模式的实施 ,大大提高了产房医疗质量。
To establish a set of brand-new responsible obstetrical model in delivery rooms and to reform the traditional obstetrical model that have been set up for several decades, so as to ensure the safety of both mothers and babies in the course of delivery and to enhance the quality of medical and nursing services. The obstetrical model of 'whole course special responsible accompany delivery' have carried out in delivery room,and the working qualities of delivery room under the new and traditional obstetrical models were compared with each other. Results: after the new obstetrical model was implemented, the vaginal perverse labor (application of vaccum extractor or forceps) rate was lowered. Patients were without prolongation of latency, stagnation of the active stage, and with no prolongation of the second stage of labor. The rate of prolonged labor among them was zero. The incidence of neonatal asphyxia, and endotracheal intubation rate of the newborns were remarkably decreased compared with those under the traditional model. There were statistical significance compared with traditional obstetric model in terms of the laceration of the cervix, the third degree laceration of perineum, postpartum hemorrhage, and the infection rate of perineal wound. Since the new obstetrical model was carried out, it has already improved the working quality of the delivery room dramatically.
