摘要
目的 了解基质金属蛋白酶 2 (mmp 2 )与抑癌基因 (nm2 3)对二型二期原发性肝癌术后预后的影响。方法 对mmp 2、nm2 3在二型二期原发性肝癌组织中的表达进行免疫组化检查 ,并将检测结果进行统计分析。结果 mmp 2在成活组患者中表达为阴性、弱阳性、阳性、强阳性分别为1、6、10、0例 ;在死亡组患者中分别为、0、2、4、7例 ;nm2 3在成活组患者中表达为阴性、弱阳性、阳性、强阳性分别为 1、6、4、6例 ;在死亡组患者中分别为 0、9、4、0例。mmp 2、nm2 3在术后有癌转移的二型二期原发性肝癌癌组织中的表达 ,弱阳性到阳性组分别为 8、1例 ,强阳性组分别为 0、7例。mmp 2表达高低与癌胚抗原 (AFP)阴阳性及肿瘤的大小无关 ;nm2 3表达高低也与肿瘤的大小无关 ,但其与AFP的阴阳性间存在统计学差异。结论 mmp 2在HBsAg阳性的二型二期原发性肝癌中表达的强弱对术后患者预后的判断优于nm2 3基因 ,而两者对肝癌病人术后的癌转移倾向有协同的预示作用。nm2
Objective To determine the correlation of matrix metalloproteinase-2(mmp-2) and nm23,and their relative importance to the prognosis and metastasis of primary hepatocellular carcinoma(PHC).Methods Samples from 30 patients operated on for PHC of BsAg positive,who had liver cirrhosis,were examined by immunohistochemistry.All PHC patients were of pattern Ⅱ stage Ⅱ.Results 30 tumour tissue specimens were tested.In the patients with better prognosis,expression of nmp-2 was positive in 10 cases,mild positive in six cases,negative in one;expression of nm23 was negative in one case,mild positive in six cases,positive in four cases,highly positive in one.Among the dead groups,expresion of mmp-2 was positive in four cases,mild positive in two cases,highly positive in seven cases;expression of nm23 was mild positive in nine,positive in four.There was no significant correlation among the expression of mmp-2,AFP and tumour size,nor was any significant correlation between the expression of nm23 and tumour size.However,they had significant correlation between the expression of nm23 and AFP,though its clinical meaning is still unknown.Conclusion The results suggest that expression of mmp-2 is better than that of nm23 in judging the prognosis of HCC.Both the expression of mmp-2 and nm23 have correlation to the metastasis of the patients of PHC with liver cirrhosis and positive HBsAg.
