摘要
目的 观察张力带钢丝内固定术固定治疗重度肩锁关节脱位的疗效。方法 将 2枚克氏针交叉由肩峰通过肩锁关节面穿入锁骨外端 ,将钢丝在肩锁关节表面作“8”字交叉后绕过针尾拧紧加压打结固定。结果 2 1例术后随访 1～ 6年 ,按优、良、差标准评定疗效 ,优 18例 (85 4 % ) ,良 3例(14 6 % )。未发生克氏针或不锈钢丝松动、滑出等并发症 ,肩关节功能恢复正常。结论 该方法使肩锁关节面对合紧密、牢固 ,为肩锁韧带、喙锁韧带、关节囊、肌筋膜、肌肉的修复创造了条件 ,手术简单 ,损伤小 。
Objective To observe the results of the internal fixation of tension band wiring for the treatment of severe acromioclavicular joint dislocation. Methods 2 Kirschner wires were penetrated intersectly through acromioclavicular joint via acromion and to the upper part of clavicula,the wire on the surface of the acromioclavicular joint was tightened, pressed, tied and fixed by the end of the stitch after it was intersected as a figure of 8. Results 21 cases were followed up for 1 to 6 years. It was found that 18 patients were classified as excellent (85 4%) and 3 as good (14 6%). No Kirschner wire or stainless steel wire became loose or slippery, the function of the shoulder joint was recovered. Conclusion The method makes the acromioclavicular joint healed firmly, inseperable, which creats conditions of acromioclavicular ligament, Caldani's ligament, joint capsule, musculofasciae and musculi repaired, the operation became simple, and little side damage is produced. The function of shoulder joint can be recovered with satisfied results.
