克拉霉素治疗呼吸道感染31例 被引量:2

A Clinical Observation on the Therapeutic Effect of Clarithromycin in the Treatment of Respiratory Tract Infection

摘要 目的 :评价克拉霉素治疗中、重度呼吸道感染的临床疗效及安全性。方法 :采用多中心开放式法 ,选取中、重度呼吸道感染患者 ,使用注射用水配制克拉霉素原液治疗 3 1例患者为治疗组 ,使用 0 .9%氯化钠溶液配制克拉霉素原液治疗的 79例患者为对照组 ,采取相同方案给药治疗。结果 :治疗组有效率 92 .8% ,痊愈率 64 .3 % ,虽均高于对照组 (有效率 82 .9% ,痊愈率 44.7% ) ,但差异无显著性。治疗组细菌清除率 1 0 0 .0 % ,对照组细菌清除率 85.7% ,两者差异有显著性 (P <0 .0 5)。治疗组注射部位的疼痛和静脉炎总发生率较对照组明显下降 (P <0 .0 5)。结论 :克拉霉素有较好的抗菌活性 ,无明显胃肠道副作用 ,可作为院外呼吸道感染 (特别是经验治疗 )较为理想的药物。在临床应用中先采用灭菌注射用水加入粉剂小瓶配制原液 ,再加入静脉滴注补液中 ,可以减少注射部位疼痛和静脉炎的发生率。 Objective:To evaluate the clinical therapeutic effectiveness and safety rate of clarithromycin in the treatment of moderate and severe infections of the respiratory tract. Methods:28 patients with moderate/severe infections of the respiratory tract chosen by the multicentric open method were included in the treatment group. For these patients, stock solution was prepared by dissolving the clarithromycin powder in aqua pro injectione before reconstituting the solution for intravenous instillation. 76 patients in similar condition suljected to the same therapeutic plan served as controls. For them, 0.9% sodium chloride solution was used instead to dissolve the powder in the preparation of the stock solution. Results:The effective and cure rates in the treatment group were 92.8% and 64.3%, respectively, both higher than those (82.9% and 44.7%) of the control group. The differences, however, were statistically insignificant. The rates of bacteria eradication in the treatment group and control group were 100% and 85.7%, respectively, the difference being significant(P<0.05). The incidence of pain and phlebitis at the injection sites was obviously lower in the treatment group than in the control group(44.3% vs 22.6%, P<0.05). Conclusion: Clarithromycin has a good antibacterial activity. It is devoid of apparent gastrointestinal side effects and is therefore a relatively ideal remedy for extra-hospital treatment (in particular empirical) of respiratory tract infections.The use of aqua pro injectione instead of 0.9% Nacl solution to dissolve the drug powder in the preparation of the stock solution which is to be added to the infusion fluid seems to be able to reduce the incidence of pain and phlebitis at the sites of injection.
