摘要
为探讨陈旧性跟骨骨折手术治疗方法选择的客观依据 ,对 72例陈旧性跟骨骨折 ,分别采用跟骨外侧骨突切除及腓骨肌肌腱松解术 (A组 )、距下关节融合及植骨术 (B组 )、跟骨截骨矫形及距下关节融合植骨术 (C组 )、三关节融合术 (D组 )四种方法治疗。术后经 1～ 6年随访 ,总优良率 87%。认为术前应对患足畸形、疼痛部位及X线片细致检查分析 ,拟定治疗方案 ,以免加重患足创伤 ,使手术取得最佳效价比。其中矫正跟骨畸形 ,尽量恢复足纵弓 。
In order to explore the objective basis of selecting therapeutic method in malunited calcaneal fracture. Seventy two cases of malunited calcaneal fracturewere treated in four ways. A: the external calcaneal osteoectomy and musculus peroneus myotenontoplasty. B: subtalar arthrodesis and grafting. C: calcaneal osteocamptomy and sutalar arthrodesis. D: three joint arthrodesis. The postoperative follow up period extended from 1 to 6 years. Eighty seven percent of the cases achieved satisfactory results. The authors concluded that selecting therapeutic method should depend on the examination of deformity of the foot, ache place of the foot and X ray diagnosis of the foot preoperatively in order to avoid aggravation trauma and achieve the best titer. The following points are very important: to try to correct the deformity of the calcaneal, to try to resume the arch of the foot, to try to resume biomechanic construction of the foot.
