脑梗塞病人血浆ET水平与血液流变学关系的研究

摘要 目的 探讨脑梗塞(CI)病人血浆内皮素(ET)与血液流变学指标的相关性。方法 测定49例CI病人和29例健康人ET、Na^+-K^+ATPase、Ca^(2+)-Mg^(2+)ATPase及血液流变学等指标。结果 CI组:ηb、ηp、ηre、HCT、K值、EAI、TK与ET呈明显正相关,与Na^+-K^+ATPase、Ca^(2+)-Mg^(2+)ATPase活力呈明显负相关;ET与Na^+-K^+ ATPase、Ca^(2+)-Mg^(2+)ATPase活力呈明显负相关(r=0.21、-0.27,P均<0.05);ηb与ET关系最为密切(r=0.43,P<0.01)。结论 ①ET、Na^+-K^+ATPase、Ca^(2+)-Mg^(2+)ATPase活力变化能引起血液粘滞性改变。②它们对CI的血液粘滞性的影响是多途径的。ET主要是通过收缩血管,改变红细胞膜Na^+-K^+泵、Ca^(2+)泵的活性,导致红细胞变形能力下降。③采用保护血管张力的药物和稀释血液等综合治疗,能有效地防治脑动脉粥样硬化和CI。 Objective To explore the relationships between endothelin (ET) and the factors of hemorheology in patients with cerebral infarction. Methods ET, Na+-K+-ATPase, Ca2+-Mg2+-ATPase and hemorheology were detected in 49 cases with cerebral infarction and 29 healthy subjects. Results b,p, re, HCT, EAI, TK were positively correlated with ET level of the plasma (P<0.01) and negatively with the activities of Na+-K+-ATPase and Ca2+-Mg2+-ATPase (P<0.01). The phasma ET level was in negative correlation with the activities of Na+-K+-ATPase and Ca2+-Mg2+-ATPase (r=-0.21, -0.27, P<0.05) in the patients with cerebral infarction. The relationship between b and ET (r=0.43, P<0.01) was more close than that between ET and any one of the other factors of hemorheology. Conclusions ①The changes of the ET, Na+-K+-ATPase and Ca2+-Mg2+-ATPase could alter the hemorheology in the patients with cerebral infarction and influenced hemorheology in various ways. ET led to the decline of erythrocyte deformability probably through contracting the blood vessels and changing the activities of the Na+-K+ and Ca2+ pump in the erythrocytic membrane.②Clinically, atherosclerosis and cerebral infarction probably can be prevented and treated by agents such as relaxing the blood vessels and diluting blood.
