Immunohistochemical Study of rasP^(21) and GST-π Expressions in Gastirc Precancerous Lessions and Adenocarcinomas

摘要 目的:观察rasP21、GST-π在胃癌及癌前病变组织中的表达,探讨复合表达在胃粘膜癌变过程中的意义。方法:应用免疫组化(S-P)法测定40例肠上皮化生(不全结肠型),76例异型增生(轻度20例,中度34例,重度22例)和42例胃癌组织中rasP21、GST-π的表达及复合表达情况,以10例萎缩性胃炎胃粘膜做对照。结果:(1)rasP21、GST-π在萎缩性胃炎中无阳性表达,在肠上皮化生、异型增生及胃癌中表达率及表达强度逐渐增高(P<0.05或0.01);在轻、中、重度异型增生中表达率及表达强度逐渐增高(P<0.05或0.01);在重度异型增生中表达率及表达强度与胃癌无显著性差异(P均>0.05)。(2)rasP21、GST-π在肠上皮化生、异型增生及胃癌中复合表达率逐渐增高(P<0.01);在轻、中、重度异型增生中复合表达率逐渐增高(P<0.05);在重度异型增生与胃癌的复合表达率无显著差异(P>0.05)。结论:rasP21、GST-π均与胃粘膜癌变过程有关,联合检测对胃癌的早期诊断有重要意义。 Purpose: In order to diagnose gastric carcinomas earlier, we studied the expressions of rasP21 protein and placental form of glutathione stransferase (GST-π) in gastric precancerous lessions and adenocarcinomas, to investigate the coherent relation and significance of the two markers in the process of gastric mucosal carcinogenesis. Methods: Expression of rasP21 protein and GST-π were examined in 40 intestinal metaplasia (incomplete colonal type), 76 gastric mucosal atypical hyperplasia(slight 20, modrate 34, severe 22) and 42 gastric adenocarcinomas by immunohistochemical technique, with S-P method using monoclonal anti-rasP21 and anti-GST-π antibodies, compared with 10 atrophic gastritis membrane. Results: (1) rasP21 and GST-π positive staining rate and intensity were gradually increased among intestinal metaplasia, atypical hyperplasia and gastric adenocarcinomas, respectively ( P <C 0. 05 or 0. 01), but not expressed in atrophic gastritis membrane. Positive staining rate and intensity were gradully increased also among slight, moderate and severe atypical hyperplasia, respectively(P<0.05 or 0.01), but there were no significant differences of positive staining rate and intensity between severe atypical hyperplasia and gastric adenocarcinomas(P >0.05). (2)rasP21 and GST-π co-expression rate were gradually increased among intestinal metaplasia, atypical hyperplasia and gastric adenocarcinomas (P < 0. 01), and also among slight, modrate and severe atypical hyperplasia, but there were no significant differences of positive staining rate and intensity between severe atypical hyperplasia and gastric adenocarcinomas( P >0. 05). Conclusions: rasP21, GST-π participate in the process of gastric mucosal carcinogenesis, and detection of the co-expression have a significant value in the diagnosis of early gastric carcinomas.
