胆结石患者胆汁细菌培养及药敏分析 被引量:6

摘要 目的 分析相隔近 8a的两组胆结石患者胆汁细菌培养与药敏的结果。方法 对 1 989- 1 0~ 1 994- 0 32 46例和 1 998- 1 0~ 2 0 0 0 - 0 598例两组胆结石患者胆汁需氧菌培养与药敏的结果进行分析对比。结果 前组 2 46例胆汁培养有2 2 8例培养阳性 ( 92 .7% ) ,共培养出需氧菌 2 30株 ,其中 G- 杆菌 2 1 2例 ( 92 .2 % ) ,G+球菌 1 8例 ( 7.8% )。后组 98例胆汁培养有 47例培养阳性 ( 4 8.0 % ) ,其中 G- 杆菌 35例 ( 74.5% ) ,G+球菌 1 2例 ( 2 5.5% )。前组胆汁培养细菌敏感性最高的有头孢三嗪、丁胺卡那霉素、头孢哌酮、头孢噻污 ,分别为 94.4%、91 .9%、91 .1 %、91 .1 %。后组胆汁培养细菌敏感性最高的有丁胺卡那霉素、氨苄青霉素 /舒巴坦、亚胺培南 ,分别为 87.2 %、84.6 %、82 .9%。结论 胆结石胆道感染 G- 杆菌为主 ,对 G- 杆菌首选丁胺卡那霉素 ,对 G+球菌首选氨苄青霉素 /舒巴坦、氨苄青霉素等廉价的药物为宜。 Objective To understand the change of main bacteria in biliary infection and their sensitivity to antibiotics in the different time.Methods 344 patients was divided into two groups:the early group came from October 1989 to March 1994,and the later group from October 1998 to May 2000.The results of biliary bacterial culture in the two groups were analyzed.Results 228 cases of biliary bacterial culture were positive in the early group, including 212 cases with gram negative bacillus and 18 gram positive coccus, and their sensitivity rates to ceftriaxime, amikacin and cefoperazone were higher in the early group.47 cases of biliary bacterial culture were positive in the later group, including 35 cases with gram negative bacillus and 12 gram positive coccus, andtheir sensitivity rates to amikacin, ampicllin/sulbactam and impenem were higher.Conclusion Gram negative bacillus is the major pathogenic microbes in biliary infection.Amikacin is the ideal choice for gram negative bacillus, and ampicillin/sulbactam or ampicillin is the ideal choice for gram positive coccus which is cheap.
