摘要
反相高效液相色谱法同时测定苯妥英、酰胺咪嗪和苯巴比妥血浓度,其平均回收率依次为92.9%,90.4%及97.0%;日内误差<6%,日间误差<8%。用于16例合并应用两种抗癫痫药物治疗患者血浓度监测及药物相互作用研究。发现8例临床无效患者皆因血浓度偏低,1例患者出现中毒症状(合用苯妥英及苯巴比妥)其苯妥英血浓度达30.4μg/ml,为临床及时调整剂量提供依据。同时发现两药合用产生复杂的药物相互作用。
A simple and sensitive reversed-phase high performance liquid chromatographic methon
for the simultaneous assay of phenytoin,carbamazepine and phenobarbital in serum has been
developed.The average recoveries were 92.9%,90.4% and 97.0% respectively.The coefficient
of diurnal variation was less than 6%,whereas that from day-to-day was less than 8%.
We applied this method to therapeutic drug monitoring of 16 patients taking the two antiepileptic
drugs,and to investigate drug interaction.We found in 8 patients the treatment failed owing
to low concentration and one patient became toxic because of high phenytoin concentration
of 30.4μg/ml(using phenytoin and phenobarbital together).Thus,the results serve to
provide a basis of dose-adjustment to the clinic and it is applicable in the study of complex
drug interaction due to combination use of two or more drugs.
