摘要
目的 :评价股骨头缺血性坏死的介入治疗方法及效果。方法 :采用 Seldinger′s穿刺技术将5 .0 F Cobra导管首先超选至旋股动脉灌注药物 ,然后再超选至髂内动脉的臀下动脉或闭孔动脉分支附近 ,留置导管连续 3d经微量泵灌注溶栓扩血管药物 ,治疗 30例患者 (共计 5 1个股骨头 )。结果 :介入治疗后 ,疼痛消失或明显缓解率为 92 .2 % ,关节功能改善率为 87.5 % ,股骨头区血管数目增多 ,空白区消失 ,实质期染色扩大增浓 ,髋关节周围侧支循环建立 ,定期复查骨质有不同程度的修复。结论
Objective: To study the method and efficacy of interventional therapy for avascular necrosis of femoral head. Methods: A 5.0F Cobra duct was first superselected into circumflex artery by applying Seldinger's technique and vasoactive drugs were perfused, then into inferior gluteal or obtruator artery that are the branches of internal iliac artery. Dissolving and dilative drugs were perfused through micro artery pump linked to the duct for three days on 30 cases (total 51 heads of femur). Results: Following interventional treatment, there were marked improvement of clinical symptoms,with relief or disappearance of pain (92.2%) and functional improvement in hip joint (87.5%) . Increase of vessel numbers, disappearance of blank space, richness of parenchmal staining and establishment of collateral circulations were found around the hip joint. Repairment of osteonecrosis at different degrees was observed on regular reexamination. Conclusion: Interventional therapy for ANFH had a significantly therapeutic effect after short term follow up.
