螺旋CT对肠系膜上动静脉的研究及临床意义 被引量:5

A Study on SMV and SMA with SCT and its Clinical Significance

摘要 目的 准确认识肠系膜上动脉 (superiormesentericartery ,SMA)、肠系膜上静脉 (superiormesentericvein ,SMV)横断面解剖 ,及其与胰腺钩突、十二指肠水平部解剖关系。方法 对行上腹部螺旋CT(spiralCT ,SCT)动脉期及门脉期双期扫描的 10 0例正常成人 ,在门脉期图像上 ,先确定呈苹果状突起的肠系膜上动脉与腹主动脉交界平面作为定位平面 ,以其下方 1~ 2cm作为测量与分析平面 ,对SMA、SMV横断位大小、形态、位置及有关间距加以测量分析 ,并对胰腺钩突及十二指肠与它们的关系进行了观察。结果 SMA、SMV多均呈管壁光滑的圆形 (83% ) ,只有少部分SMV呈椭圆形 (17% )。管径SMV绝大多数大于SMA(95 % ) ,少数相等 ,未见一例相反。两者关系方面 ,绝大多数 (94% )SMV位于SMA之右侧 ,少数情况下两者呈前后平行关系 (6 % )。SMV偏前者 (5 1% ) ,SMA偏前者 (2 4% ) ,两者左右并行 (19% )。两者中心间距平均 15 .31mm左右。SMV与正中矢线距离平均 18.0 9mm左右。,SMV、SMA距椎体前缘水平线之垂直距离平均 41.30、40 .10mm。而胰腺钩突全部位于SMV与下腔静脉之间。 2 0例胰腺病变者 ,主要表现为血管周围脂肪模糊不清 ,血管受压、变形、推移及包绕破坏。结论 SCT增强检查可清楚显示SMV、SMA及其与周围脏器的关系 。 Objective To study the anatomic locations of superior mesenteric artery (SMA) and superior mesenteric vein (SMV) by SCT and to explore its clinical significance by recongnizing these relations with the surrounding organs including the uncinate process of pancreatic head, the third duodenum, etc..Methods 100 normal cases were performed with enhancement SCT. Imaging material of parenchymal enhancement from them was selected. First, we confirmed the level where SMA arises from the aorta, then take the level which is about 1.5 mm lower the aorta as our measurement and observation level.Results 83% of SMA and SMV were round in shape, only 17% of them were ellipse in shape in cross sections. Most of the lumen diameters of SMV were larger than that of SMA , little of them were equal,. and no exception was found contrarily. As for the anatomic locations between themselves, 94% of SMV lied on the right of SMA, only 6% of them lied parallelly in anterior and posterior directions. 51% of SMV were anterior to SMA,24% were posterior to it, and 19% ran parallelly. The average distance between them was 15.31 mm,; the average distance between SMV and the central sagittal line was 18.09 mm.; the average vertical distances between both SMV and SMA with the horizontal line of the anterior verge of spinal cord were 41.30±8.90 mm and 40.10±8.20 mm, respectively. All uninate process of pancreatic head extended between the inferior vena cava and SMV. In some cases, the third duodenum was dorsal to SMV and SMA.Conclusion SCT contrast examination can clearly demonstrate SMV and SMA; and their relations to the surrounding organs are helpful in diagnosis and especially in locating the uncinate process of the pancreatic head.
