摘要
目的 比较老年性痴呆 ( AD)和血管性痴呆 ( VD) 18F-FDG PET显像特征 ,为诊断和治疗提供帮助。方法 将受检者分为 3组 :其中 AD组 1 4例 ,VD组 6例 ,正常对照组 6例。静脉注射 18F-FDG1 85-3 70 MBq,4 0 min后采用 PET扫描仪行脑显像。结果 正常对照组双侧各脑叶和小脑的葡萄糖代谢分布对称。VD组 6例 ,病灶呈非对称性分布于脑叶皮层 ,其中 4例病灶波及多叶及丘脑和基底节 ,并出现交叉性小脑失联络。AD组的双侧顶叶、颞叶的代谢明显减少 ( P <0 .0 0 1 ,P <0 .0 5)。结论 AD和 VD的 PET显像各有其特点 ,PET能敏感地区分他们。
Objective To compare PET image characteristic of Alzheimer's disease with those of vascular dementia to provide help for clinic treatment and diagnosis of them.Methods Of 26 cases were divided into three groups, such as 14 for AD group, 6 for VD group, and 6 for normal control group. After 40 minutes of intravenous administration of 18 F FDG,PET scan was performed. In the mean time, examineer's blood glucose must be controlled less than 6.7 mmol/L.Results Compared with the control, there was symmetric hypometabolism in the parietal lobes for 8 cases; there was symmetric hypometabolism both in the parietal lobes and in the temple lobes for 6, of which one referred to the frontal in AD group. In VD group, there was asymmetric hypometabolism in brain lobes for 6 cases, of which 4 referred to basis ganglia and hypothalumus , and crossed cerebellar diaschisis occurred. Conclusion There is evident difference of 18 F FDG PET image between Alzheimer's disease and vascular dementia ,PET can differentiate them sensitively.
