取样方法和昼夜节律对卡马西平唾液浓度的影响 被引量:8

摘要 目的 研究唾液取样方法和昼夜生物节律对唾液中卡马西平 (CBZ)浓度的影响 .方法  1以自然取样法、棉球法和柠檬酸法顺次收集 12例癫痫患者唾液 ,同时收集血液样品 ;2以棉球法对 8例患者在 2 4h内的 4个时间点分别收集唾液并同时收集血液 .HPL C法测定唾液和血液样品中 CBZ浓度 ,并对两者之间的相关性进行统计学比较 .结果 卡马西平在唾液中的浓度分别为 :自然取样法 2 .81m g· L- 1 ,棉球法 2 .0 6 mg· L- 1 ,柠檬酸法 2 .2 2 mg· L- 1 ;唾液药物浓度与血药浓度之间的相关系数分别为 :自然取样法 0 .85 3,棉球法 0 .90 4,柠檬酸法 0 .90 6 .不同时间点 (7:0 0 ,11:0 0 ,15 :0 0和 2 1:0 0 )的唾液药物浓度与血药浓度之比 (S/P)分别为0 .335 ,0 .32 1,0 .30 9和 0 .35 8;相关系数 (r)分别为 :0 .85 ,0 .93,0 .92和 0 .98.结论 棉球法和柠檬酸法优于自然取样法 ,CBZ在唾液中的分布受昼夜生物节律的影响 ,收集时间宜选择 11:0 0 ,15 :0 0和 2 1:0 0 . AIM To investigate the effect of collection methods and diurnal rhythm on the carbamazepine (CBZ)level in saliva. METHODS ①The saliva samples were collected by three methods (Nature, Cotton ball and Citric acid) in turn from 12 epileptic patients with blood samples collected simultaneously. ② The Cotton Ball was used to collect 8 patients's saliva in 4 time points within 24 hours with blood samples collected simultaneously. The CBZ levels in saliva and serum were measured by HPLC; then compared the correlationship of them between saliva and serum statisticly. RESULTS The CBZ concentrations in saliva gathered by different methods were 2.81 mg·L -1 (Nature), 2.06 mg·L -1 (Cotton ball) and 2.22 mg·L -1 (Citric acid); the correlation coefficients (r) between saliva and plasma was 0.853, 0.904, 0.906 respectively. Multi time points detection result showed that the concentration of CBZ was changed in one day, the saliva: plasma concentration ratios (S/P) at 7:00, 11:00, 15:00, 21:00 were 0.335, 0.321, 0.309 and 0.358. The correlation coefficients (r) were 0.85, 0.93, 0.92 and 0.98. CONCLUSION The methods of Cotton ball and Citric acid is better than Nature, the CBZ concentration in saliva is influenced by the diurnal rhythm; 11:00, 15:00 and 21:00 is the best time points for saliva collection.
