摘要
目的 :探讨浅静脉干在局部皮瓣成活中的作用。 方法 :在猪的大腿及腹部设计蒂在近端的长宽比为 4:1的局部皮瓣 (2 0× 5 cm) ,按静脉干的处理不同分三组 : 组包含浅静脉干及其周围组织 , 组将浅静脉干在皮瓣蒂部栓塞 , 组将浅静脉干及其周围 0 .5 cm内的软组织结扎切断。术后即刻 ,2 4h,48h用激光多普勒血流探测仪检测皮瓣蒂部 ,中部 ,远端的微循环血流情况 ,7天观察各组皮瓣成活情况 ,术后 7天切取 、 组皮瓣 ,自蒂部用 10 %硫酸钡钡餐乳剂行静脉干逆行造影。 结果 : 、 组间蒂部不同时期微循环血流灌注速率 ,峰值均数比较无显著差别 (P>0 .0 5 )。 结论 :局部皮瓣带上浅静脉干不仅可以通过其本身的滋养系统为皮瓣提供部分血供 ,同时其与周围组织的广泛联系亦有助于皮瓣的引流 ,尤其是浅静脉干的引流作用更重要 。
Objective:To study the role of superficial venous trunks(SVT) in the survival of flaps Methods:Proximally based local flaps were designed in the thighs and abdomens of pigs The local flaps whose length width ratio was 4:1(20×5cm)were divided into 3 groups according to the SVT Group 1:contained the SVT and the surrounding tissue;group 2:the SVT were embolized in the pedicles;group 3:the SVT and the surrounding tissue within 0 5 cm were ligated and amputated A laser doppler haemometer was used to measure the arterial blood of the pedicles,middle parts and ends of the flaps immediately affer operations 24 hours,48 hours postoperatixely,the saurvival of flaps were resoarched in 7 days postoperatively,the flaps of group 1 and 2 were resected at the same time, and 10% BaSO 4 was used to perfused from pedicles Results:There was no significant difference in the ends between group 1 and 2(P>0 05);but there was significant difference in the ends between group 1and 2in24 hours after operations(P<0 05),which result into significant difference of survival between group 1 and 2(P<0 05) There was no significant difference of survival between group 2 and 3(P>0 05) Conclusion:The SVT can not only applied the local flaps with some arterial blood through nourishing system of themselves,but also improve the venous drainage of local flaps The reverse flow of venous blood was not documented in the experiment.
