螺旋CT三维和多平面重建在跟骨骨折中的应用 被引量:9

摘要 目的 探讨螺旋CT三维和多平面重建(MPR)在跟骨骨折中的临床价值。方法 分析28例30个跟骨骨折的轴位CT、MPR及表面重建(SSD)CT的表现,对骨折进行分型,并选择合适的治疗方案。结果 30个跟骨骨折中,骨折涉及距下关节6个跟骨,涉及距下关节24个跟骨。22例骨折涉及距下关节者进行手术治疗,16例术中给予植骨,手术结果证实了MPR和SSD显示的骨折情况,并取得良好疗效。MPR能准确显示骨折的部位、类型、严重性、移位的程度及相应的软组织损伤;SSD有助于骨折块的立体定位,术前正确评估骨关节损伤和塌陷的程度。结论 螺旋CT三维重建和MPR对术前骨折的分类、治疗方案的选择及估计预后有重要价值。 Purpose To evaluate the clinial application value of three - dimension(3D)and multiplanar reconstruction (MPR)with sprial CT in calcaneal fractures.Methods CT findings on axial,MPR and surface shaded display(SSD)images in 28 patients with 30 calcaneal fractures were analyzed, and the fractures were classified and appropriately treated. Results In 30 calcaneal fractures, 6 cases didn't involve subtalar joints, and the rest 24 cases involved subtalar joints. 22 cases involvement subtalar joints underwent operation, 16 of these had graft. The lesions of calcaneum on MPR and MPR were demonstrated correctly by operation. MPR accurately revealed the location, type, severity, the displacement of fragments and the change of soft tissue. SSD helped to improve visualization of the complex bone spatial, and well evaluated preoperatively the degree of joint injury and depression. Conclusion Sprial CT 3D and MPR play an important role in the preoperative classification of calcaneal fractures, and are useful for the selection of surgical approaches, and for the evaluation of prognosis.
